SN instructed in new medication Apresoline ( Hydralazine ), its uses as a vasodilator to decrease BP and common side effects such as; Headache, anorexia, dizziness, hypotension, nausea, vomiting, diarrhea, palpitations, tachycardia, angina pectoris. SN instructed patient and caregiver on how to stop a nosebleed. Make a thumbs up with hand on same side as nose bleed. Press side of nose that is bleeding closed and tilt head slightly down to prevent blood from going down into throat. Stay still for 5-10 minutes then gently release. Refrain from blowing nose or putting a tissue in nose x 24 hours. If bleeding does not stop seek medical attention.
Instructed what can you do to manage shortness of breath (SOB) use devices to help you save your energy some devices can help to cut down on the work your body needs to do in order to breath. Use pillows to raise your head when lying down. Use a reclining chair with a footrest when sitting. Use a walker or wheelchair if you need them.
Instructed in the depression's symptoms such as constant feelings of sadness, irritability or tension, decreased interest or pleasure in usual activities or hobbies,loss of energy, feeling tired despite lack of activity, a change in appetite with significant weight loss or weight gain, difficulty sleeping, early morning awakening or sleeping too much, restlessness or feeling slowed down decreased ability to make decisions or concentrate, feelings of worthlessness, hopelessness or guilt, thoughts of suicide or death.

Instructed patient to flush all expired medication down the toilet.
Instructed patient to take his/her pills with a full glass of water and drink all the water even if the pill went down with the first swallow.
