Why is it important to do active ROM exercises? ROM exercises are very important if you have been ill, injured, or must stay in bed or in a wheelchair. Active ROM exercises help build up or keep your muscles as strong as possible. They help keep your joints flexible ( bending and moving in the right way ). Doing ROM exercises will help keep good blood flow going to the joint area that is being exercised. They may help prevent blood clots. Exercising is natural and needed for the body to stay well. Your heart pumps stronger and easier when you exercise regularly. You also breathe quicker and more often while exercising. This helps prevent infections that happen if you do not breathe or cough deeply enough. With your heart and lungs working stronger, more blood gets to the muscles, bones, and skin. Increasing blood flow brings more oxygen and other good things to your body tissues and organs. Walking and other exercises help make your BMs and urine more regular. Exercising can also make you feel better, more relaxed, and happier.

SN instructed patient / caregiver that the stroke occurs when a clot or a torn blood vessel in the brain stops blood from reaching a part of the brain. Damage to that part of the brain from lack of blood and oxygen can cause various signs and symptoms of stroke, such as facial drooping, numbness and paralysis. Although anyone can have a stroke, your risk increases if you're male, over age 65, or have one of these conditions: high blood pressure, high cholesterol, heart disease, or diabetes. Being overweight, smoking, abusing drugs or alcohol, and taking birth control pills increase risk, too. African - Americans, people who are Hispanic or Asian, and those with a close relative who's had a stroke are also at higher risk.

Spontaneous coronary artery dissection ( SCAD ) Teaching 2134

SN instructed patient on spontaneous coronary artery dissection ( SCAD ) and stent placement. A stent is a small rod made out of metal or fabric. It is inserted into a coronary artery when there is a blockage or when the artery is narrow. A coronary artery is a blood vessel that carries oxygen to the heart muscle. You received a stent due to coronary artery disease ( also called SCAD). That is caused by a thickening of the inside walls of the coronary arteries. This thickening is called atherosclerosis. A fatty substance called plaque builds up inside the thickened walls of the arteries, blocking or slowing the flow of blood. If your heart muscle doesn't get enough blood to work properly, you may have angina ( chest pain ) or a heart attack. The stent will help hold the artery open so blood can flow and help keep the artery from narrowing again. You will need to take medicine that keeps the stent open and blood flowing. It is very important to take the medicine exactly how your doctor tells you to take it. In addition to taking medicine, the best way to keep your heart healthy is to maintain a healthy lifestyle. Quit smoking. Make sure you keep your blood pressure and cholesterol at healthy levels. Exercise as recommended by your doctor. Eat a heart - healthy diet. Patient verbalized understanding well.

Sn instructed patient about diabetic wounds / ulcers have poor or delayed healing. Healing problems are caused by the peripheral arterial diseases and peripheral neuropathy that can occur with diabetes, wherein the small blood vessels in different parts of the body, especially in the extremities ( hands and feet ), grow narrower and reduce the blood circulation to those areas. A lack of circulation in the extremities can result in a reduced supply of oxygen and nutrients to the body tissue and nerves, which is necessary for healing. Over time, nerves in these areas may become damaged, decreasing the sensation of pain, temperature and touch, making patients vulnerable to injury.

Sn instructed on monitoring vital signs blood pressure, temperature, oxygen and weight instructed on what equipment is needed and SN will instruct on equipment logging and reporting of vital signs. Sn instructed on importance in logging vital signs and monitoring if medication is working or change of condition. Lisinopril Teaching 2219

SN instructed patient about the drug lisinopril is a drug of the angiotensin-converting enzyme inhibitor class used primarily in treatment of hypertension ACE is important because it is an enzyme responsible for producing the chemical, angiotensin II. Angiotensin II causes muscles in most arteries, including the arteries of the heart, to contract, thereby narrowing the arteries and elevating blood pressure. ACE inhibitors such as lisinopril lower blood pressure by reducing the production of angiotensin II, thereby relaxing arterial muscle and enlarging arteries. When the blood pressure is lower, the heart - including the failing heart - does not have to work as hard to pump blood. The arteries supplying the heart with blood also enlarge during treatment with ACE inhibitors. This increases the flow of blood and oxygen to the heart, further improving the ability of the heart to pump bloodSide effects include: dizziness due to a drop in blood pressure, trouble breathing, blistering, peeling, red skin rash, confusion, weakness, uneven heartbeat, trouble breathing, numbness or tingling in your hands, feet, or lips, dark urine or pale stools. SN educated patient on the importance of daily vital sign monitoring. Due to patients disease processes it is important to monitor blood pressure, weight, pulse and oxygen daily if equipment is available. SN instructed patient to weigh correctly they need to wake up, pee and than weigh daily at the same time if possible, same amount of clothing, same area. Patient is to weigh prior to eating/drinking in the morning. Notify home health or PCP if -/+ 3 lbs in a day or +/-5 lbs in a week is seen. SN educated patient to monitor blood pressure and pulse, and instructed to check before medications and if elevated/low recheck in 1-2 hours. SN educated patient to always log vitals so patients MD has a larger snap shot on what is going on.Patient/CG verbalized understanding

SN instructed on arteriosclerosis is hardening and thickening of the walls of the arteries. This condition can occur because of fatty deposits on the inner lining of arteries (atherosclerosis), calcification of the wall of the arteries, or thickening of the muscular wall of the arteries from chronically elevated blood pressure. When arteriosclerosis affects the coronary arteries that supply blood to the heart muscle, a shortage of oxygen delivered to the heart itself may lead to a heart attack.
