Patient instructed that the most common symptoms of Clostridium difficile colitis (C. diff) infection include: watery diarrhea, fever, loss of appetite, nausea, belly pain and tenderness. Instructed to clean their hands with soap and water or an alcohol-based hand rub often and every after going to the restroom.

Parkinson Teaching 1678

Patient instructed on how Levodopa and Carbidopa are used in combination to treat the symptoms of Parkinson's disease. Carbidopa prevents the breakdown of Levodopa in the bloodstream so more Levodopa can enter the brain. This can reduce some of Levodopa's side effects such as nausea and vomiting, and it may also allow your doctor to increase your Levodopa dose more quickly to find the best dose.

Coronary Artery Disease Teaching 1679

Patient instructed in Coronary Artery Disease (CAD) is when the heart vessels become narrow, which decreases the blood flow to the heart muscle. The narrowing of the heart vessels can be caused by plaque build-up from high cholesterol, fat, and calcium. The decrease in blood flow decreases the oxygenation of the heart muscle and leads to heart damage.

Congestive Heart Failure Teaching 1680

Patient instructed in congestive heart failure occurs when your heart is not able to pump enough blood to meet the needs of the rest of your body. If you have CHF, blood and extra fluid may back up into your lungs, making it difficult for you to breathe, swelling in your ankles and legs (called edema) may occur, you may experience excess fatigue with exercise.

Angina pectoris Teaching 1698

SN instructed patient and caregiver about what angina pectoris is. Angina pectoris is chest pain that is caused by insufficient oxygen to meet demands of the heart. Lack of oxygen occurs when insufficient blood flows through the coronary arteries. Stable angina usually has a precipitating cause while unstable angina can occur at rest.

SN instructed patient on low salt diet. SN instructed patient on how to read nutritional labels on commercially prepared foods. Other salt alternatives such as Mrs. Dash was encouraged. SN explained how sodium affects blood pressure and water retention.

Hyperglycemia Teaching 1730

Patient was instructed If you eat more food than is balanced with your physical activity and, in some cases, diabetes medicines, your blood glucose level may rise above your goal range. Carbohydrate-containing foods directly affect your blood glucose level after eating, so reviewing the amount of carbohydrate in your meals and snacks may be helpful in determining the cause of hyperglycemia.

Hyperglycemia Teaching 1731

Instructed patient during periods of stress, the body releases so-called stress hormones, which cause a rise in blood glucose level. In the short term, this gives the body the extra energy it needs to cope with the stress. But if a person doesn’t have adequate insulin circulating in his bloodstream to enable his cells to use the extra energy, the result will be hyperglycemia. And if stress becomes chronic, hyperglycemia can also become chronic.

Hyperglycemia Teaching 1732

SN instructed patient psychological stresses such as difficulties with relationships, job pressures, financial strain, and even concerns about self-worth can contribute to hyperglycemia. If these issues become overwhelming, decreased attention to the diabetes treatment plan may also contribute to hyperglycemia.

Hypoglycemia Teaching 1738

Patient and caregiver instructed that low blood sugar (hypoglycemia) can happen to everyone who has diabetes. Symptoms include headache, hunger, sweating, pale skin, irritability, dizziness, feeling shaky, or trouble concentrating. Always keep a source of sugar with you in case you have low blood sugar. Sugar sources include fruit juice, hard candy, crackers, raisins, and non-diet soda. Be sure your family and close friends know how to help you in an emergency.
