SN instructed patient / caregiver that the combination preparation fluticasone/salmeterol (Advair diskus) is a formulation containing fluticasone propionate and salmeterol xinafoate, used in the management of asthma and chronic obstructive pulmonary disease. Commun side effect / adverse reaction may include: Body aches or pain, choking, congestion, dryness of the throat, high-pitched noise when breathing, hoarseness, runny nose, sneezing, trouble with swallowing, voice changes. Less common side effect / adverse reaction: Cough-producing mucus, flu-like symptoms, irritation or inflammation of the eye, muscle
pain, pain or tenderness around the eyes and cheekbones, sleep disorders, stuffy nose, white patches in the mouth or throat or on the tongue.
SN instructed patient that Vesicare (solifenacin) reduces muscle
spasms of the bladder and urinary tract. It is used to treat symptoms of overactive bladder, such as frequent or urgent urination, and incontinence ( urine leakage) . The most common side effect of solifenacin are dry mouth, blurred vision, and constipation. As all anticholinergics ,it may rarely cause hyperthermia due to decreased perspiration.
SN instructed patient / caregiver that Tamsulosin is used in the treatment of difficult urination, a common symptom of enlarged prostate ( BPH ). Tamsulosin works by relaxing bladder neck muscle
s and muscle
fibers in the prostate itself and make it easier to urinate. Possible side effect may include, Higher risk of allergic reaction in those with sulfa allergies. Patients taking tamsulosin are prone to a complication known as floppy iris syndrome during cataract surgery. Severe hypotension. It also affect sexual function in men. It can cause males to experience semen entering the bladder.
SN instructed that doxycycline is a tetracycline antibiotic. It fights bacteria in the body. Take doxycycline exactly as prescribed by your doctor. Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended. Take doxycycline with a full glass of water. Drink plenty of liquids while you are taking this medicine. Most brands of doxycycline may be taken with food or milk if the medicine upsets your stomach. Different brands of doxycycline may have different instructions about taking them with or without food. Take Oracea on an empty stomach, at least 1 hour before or 2 hours after a meal.Call your doctor at once if you have: changes in your vision; severe stomach pain, diarrhea that is watery or bloody, fever, swollen glands, body aches, flu symptoms, weakness, skin rash, pale skin, easy bruising or bleeding, severe tingling, numbness, pain, muscle

SN instructed patient / caregiver on Xanax / Alprazolam Which is a medication in the class of benzodiazepines. Explained that this medication is indicated in the management of anxiety and panic attacks and that it works by acting on the CNS to decrease anxiety. Explained to patient that the side effects include: dizziness, drowsiness, lethargy, headache, rashes, nausea, vomiting. Explained that abrupt discontinuation of this medication can cause withdrawal symptoms such as: sweating, nausea, vomiting, muscle
cramps, tremors and seizures. Instructed patient to avoid ETOH, CNS depressants, and grapefruit juice while taking Xanax. Instructed to notify Physician if experiencing unrelieved anxiety or if experiencing persistence or worsening of medication side effects
SN instructed patient / caregiver that Aldactone is a potassium - sparing diuretic ( water pill ) that prevents the body from absorbing too much salt and keeps potassium levels from getting too low, treating congestive heart failure. SN instructed patient / caregiver to take Aldactone as prescribed by the doctor and do not use salt substitutes or low - sodium milk products that contain potassium, because it may increase potassium levels too much. SN explained that side effects may include mild nausea, dizziness, headache, gastric pain. SN instructed to report to physician ( MD ) if patient experiences an allergic reaction, numbness, muscle
pain or weakness, irregular heartbeats, urinating less, shallow breathing or tremors.
SN instructed patient / caregiver on medication Amikacin. SN explained that Amikacin is an anti - infective in the aminoglycoside family which is indicated as part of combination therapy for mycobacterium avium complex. SN explained that side effects include ototoxicity, nephrotoxicity, apnea and hypersensitivity. SN instructed patient / caregiver that call your physician ( MD ) if you experience to signs of hypersensitivity, tinnitus ( ringing of the ears ), vertigo ( dizziness ), muscle
weakness, twitching, feeling of fullness in the head or hearing loss.
SN instructed patient / caregiver on medication Avandaryl. Explained that it is a medication indicated for Diabetes Mellitus ( DM ) type II to decrease the blood sugar by stimulating the production of insulin and increasing receptor site sensitivity. SN explained to patient / caregiver that the side effects include dizziness, photosensitivity, low blood sugar, constipation, nausea and vomiting. SN instructed patient / caregiver to monitor blood sugar daily and to not skip or double doses. SN instructed patient / caregiver to notify physician ( MD ) if experiencing unusual weight gain, swelling of ankles, drowsiness, dyspnea ( SOB ), muscle
cramps, weakness, sore throat, rash, unusual bleeding or if bruising occurs.
SN instructed patient / caregiver on medication Clonazepam. Explained that this medication is a benzodiazepines and it works to manage panic disorder as well as muscle
convulsions and neuralgias. Explained that the side effects include behavioral changes, drowsiness, increased secretions, palpitations, constipation, ataxia and dependence. SN instructed patient / caregiver to notify physician ( MD ) if fever, clay colored stools, yellowing of skin, or behavioral changes occur.
SN instructed patient regarding medication Furosemide. SN informed patient that Furosemide is used to treat fluid retention and high blood pressure and it causes increased urination. SN instructed patient to watch out for side effects of this medication such as headache, dizziness, blurred vision, constipation, diarrhea, and numbness / burning / tingly feeling and to notify physician ( MD ) if experiencing these side effects persistently. SN informed that Lasix may deplete potassium in the body, thus needing potassium supplement or increased potassium intake ( oranges, bananas ). SN instructed patient / caregiver to report to physician ( MD ) if patient experiences vomiting, fast / uneven heartbeat, lightheadedness or increased weakness, muscle
pains, or urinating less or not at all.
