Patient was educated about potassium 10 meq 1 tablet oral daily this medication is given for treatment/ prevention of potassium depletion.
SN instructed patient about Triple antibiotic ointment Neosporin, it is an antibiotic product marketed for the prevention of infections and speeding the healing of wounds. Neosporin is recommended for burns, scratches, and minor cuts. It is most effective when affected area is cleaned before application of ointment. It is for external use only and should not go near mucus membranes such as the eyes or mouth. It is not recommended for children under the age of two. There are no known side effects when using neosporin, however users should immediately seek medical attention when experiencing hives, rashes, or itching. Any skin irritations such as pain, burning, or cracked skin that were not present prior to use of ointment must receive immediate care.
SN instructed patient that lovaza is approved in the U.S. for treatment of patients with very high triglycerides (hypertriglyceridemia). Lovaza is indicated for: Hypertriglyceridemia, used as monotherapy , or in combination with a statin for patients with mixed dyslipidemia. Also as a secondary prevention after myocardial infarction. Infrequent side effects of lovaza: burping, indigestion, rash, taste problem. Rare side effects of Lovaza: blood clotting disorder. Life threatening allergic reaction, abnormal liver function tests, Itching.
SN instructed patient / caregiver regarding medication Coumadin / Warfarin. SN explained to patient / caregiver that Coumadin / Warfarin is an anticoagulant which is prescribed to people with an increased tendency for thrombosis or as secondary prophylaxis ( prevention of further episodes ) in those individuals that have already formed a blood clot ( thrombus ). Explained that Warfarin treatment can help prevent formation of future blood clots and help reduce the risk of embolism. Informed that Warfarin will prolong bleeding time, instructed to avoid activities that increase risks of trauma. SN explained to patient / caregiver that the adverse effects of this medication includes: hemorrhage, nosebleeds and blood in urine or stool ( black tarry stools ). Instructed to notify physician if experiencing unusual bleeding while on this medication.
SN instructed patient that Prednisone is a synthetic corticosteroid drug that is particularly effective as an immunosuppressant drug. It is used to treat some types of cancer, but has significant adverse effects. It is used for many different indications including: Pericarditis, multiple sclerosis, nephrotic syndrome, to relieve the effects of shingles, lupus, myasthenia gravis, poison oak exposure, meniere's disease and as part of a drug regimen to prevent rejection after organ transplant. Possible side effect may include: Abdominal pain, peptic ulcer, painful hips or shoulders, steroid-induced osteoporosis, stretch marks, osteonecrosis - same as avascular necrosis, insomnia, severe joint pain, cataracts or glaucoma, anxiety, black stool, stomach pain or bloating, severe swelling among others. Take prednisone as ordered.
Sn instructed patient that venelex ointment contains balsam peru 87.0mg and castor oil usp 788.0mg in an ointment base of glyceryl monostearate and trypsin. It promotes debridement of eschar and other necrotic tissue. Balsam peru is a effective capillary bed stimulant used to increase circulation in the wound site area. Also, balsam peru has a mildly bactericidal action. Castor oil improves epithelialization by reducing premature epithelial desiccation and cornification. Also, it can act as a protective covering and aids in the reduction of pain. Venelex ointment is used to promote wound healing and the treatment of decubitus ulcers, varicose ulcers and dehiscent wounds. Venelex™ ointment is easy to apply and quickly reduces odors frequently accompanying a decubitus ulcer. The wound may be left open or appropriate dressing applied. Please note that wounds generally heal poorly in the presence of hemoglobin or zinc deficiency. Venelex™ ointment can relieve pain and promote healing. Venelex ointment is for external use only. Do not apply to fresh arterial clots. Avoid contact with eyes. Keep this and all other medications out of reach of children. Keep tightly closed. Use only as directed by a physician. When applied to a sensitive area, a temporary stinging may occur. Apply a thin film of venelex™ ointment topically a minimum of twice daily or as often as necessary. Wound may be left unbandaged or appropriate dressing can be applied. To remove, wash gently with an appropriate wound cleanser.
SN educated patient on sucralfate medication. Patient was instructed that this medication is used to treat and prevent ulcers in the intestines. Sucralfate forms a coating over ulcers, protecting the area from further injury. This helps ulcers heal more quickly. Patient verbalized understanding.
SN instructed patient about the drug lisinopril is a drug of the angiotensin-converting enzyme inhibitor class used primarily in treatment of hypertension ACE is important because it is an enzyme responsible for producing the chemical, angiotensin II. Angiotensin II causes muscles in most arteries, including the arteries of the heart, to contract, thereby narrowing the arteries and elevating blood pressure. ACE inhibitors such as lisinopril lower blood pressure by reducing the production of angiotensin II, thereby relaxing arterial muscle and enlarging arteries. When the blood pressure is lower, the heart - including the failing heart - does not have to work as hard to pump blood. The arteries supplying the heart with blood also enlarge during treatment with ACE inhibitors. This increases the flow of blood and oxygen to the heart, further improving the ability of the heart to pump bloodSide effects include: dizziness due to a drop in blood pressure, trouble breathing, blistering, peeling, red skin rash, confusion, weakness, uneven heartbeat, trouble breathing, numbness or tingling in your hands, feet, or lips, dark urine or pale stools. Pt verbalized understanding.
Instructed in new medication Pred Forte to manage inflammation of palpebral and bulbar conjunctiva, cornea and anterior segment of globe. In addition, warned of possible S/E, such as, increased intraocular pressure, thinning of cornea, interference with corneal wound healing, increased susceptibility to viral or fungal corneal infections, corneal ulceration, discharge, discomfort, foreign body sensation, glaucoma exacerbation, cataracts, visual acuity and visual field defects, optic nerve damage with excessive or long-term use, systemic effects, and adrenal suppression with excessive or long-term use. Instructed to wash hands before and after installation and warned not to touch tip of dropper to eye or surrounding area. Apply light finger pressure on lacrimal sac for 1 minute after installation. On long-term therapy, have frequent tests of intraocular pressure. Instructed not to share drug, washcloths, or towels with family members, and notify MD if anyone develops same signs or symptoms. Stress importance of compliance with recommended therapy. Notify MD if improvement does not occur within several days or if pain, itching, or swelling of eye occurs. Instructed not to use leftover drug for new eye inflammation because serious problems may occur.
SN instructed patient / caregiver regarding medication Metoprolol. Metoprolol is used for treating high blood pressure, heart pain, abnormal rhythms of the heart, and some neurologic conditions; it reduces the force of contraction of heart muscle and thereby lowers blood pressure. Side effects to report to physician ( MD ) when taking Metoprolol including diarrhea, constipation, fatigue, insomnia, nausea, depression, memory loss, fever, lightheadedness, slow heart rate, low blood pressure, cold extremities, sore throat, and shortness of breath or wheezing. If experiencing any of these side effects or when systolic BP is >160 or < 90 and / or diastolic BP is > 90 or < 60 notify physician ( MD )
