Patient instructed Duloxetine is used to treat major depressive disorder in adults. It is also used to treat general generalized anxiety disorder. It is also used in adults to treat fibromyalgia (a chronic pain disorder), or chronic muscle
or joint pain (such as low back pain and osteoarthritis pain). Take duloxetine exactly as prescribed by your doctor. Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended. You may take duloxetine with or without food. Do not crush, chew, break, or open an extended-release capsule. Swallow it whole.
Alendronate is a bisphosphonate medicine that alters bone formation and breakdown in the body. This can slow bone loss and may help prevent bone fractures. Alendronate is used to treat osteoporosis caused by menopause, steroid use, or gonadal failure. This medicine is for use when you have a high risk of bone fracture due to osteoporosis. Common side effects may include heartburn, upset stomach, stomach pain, nausea, diarrhea, constipation, bone pain, or muscle
or joint pain. SN instructed pt to report if she experiences any of these s/sx. Pt voiced understanding.
Patient/caregiver instructed Allopurinol reduces the production of uric acid in your body. Uric acid buildup can lead to gout or kidney stones. Take once daily as ordered with a full glass of water. Medication may weaknen your immune system, avoid sick persons. Common side effects are vomiting, diarrhea; drowsiness, headache; changes in your sense of taste; or muscle
pain. Notify SN/MD if experiencing bothersome or unusual side effects.
SN instructed patient/ caregiver about Baclofen.That it is a muscle
relaxer and an antispastic agent. SN instructed to Get emergency medical help if you have signs of an allergic reaction to baclofen such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: weak or shallow breathing; confusion, hallucinations; or a seizure (convulsions). Common baclofen side effects may include: drowsiness, dizziness, weakness, tired feeling; headache; sleep problems (insomnia); nausea, constipation; or urinating more often than usual.
SN instructed about medication Trazodone that is used to treat depression. It may help to improve your mood, appetite, and energy level as well as decrease anxiety and insomnia related to depression. Trazodone works by helping to restore the balance of a certain natural chemical (serotonin) in the brain. Nausea, vomiting, diarrhea, drowsiness, dizziness, tiredness, blurred vision, changes in weight, headache, muscle
ache/pain, dry mouth, bad taste in the mouth and stuffy nose.
Instructed patient that the most commonly reported side effects of Amlodipine (which is used to treat HTN) include: headache, edema, rash, fatigue, and dizziness. Check with MD or HH nurse if any of the following side effects continue or are bothersome: acid or sour stomach, belching, heartburn, indigestion, lack or loss of strength, muscle
cramps, sleepiness or unusual drowsiness, and stomach discomfort, upset, or pain.
Instructed patient on Prednisone which is used for allergy reactions, rheumatoid arthritis among other diseases. Side effects include: swelling in hands, face and feet, increased hunger, muscle
weakness and abdominal distention. One interaction of Prednisone is it may cause hypernatremia, hypokalemia, fluid retention, and elevation in blood pressure.
Trulance (plecanatide) is used to treat chronic idiopathic constipation or irritable bowel syndrome with constipation. Trulance works by activating certain substances within your intestines that help increase fluid secretion and muscle
movement. Trulance also causes changes in the consistency of your stools.
SN instructed patient and caregiver on SIMVASTATIN. Simvastatin is a cholesterol-lowering medicine. It inhibits the production of cholesterol by the liver. High blood cholesterol is first treated with exercise, weight loss, and a diet low in cholesterol and saturated fats. When these measures fail, cholesterol-lowering medications such as Simvastatin can be added. Simvastatin is generally well-tolerated and side effects are rare. Minor side effects of Simvastatin include constipation, diarrhea, fatigue, gas, heartburn, and headache. Major side effects of Simvastatin include abdominal pain or cramps, blurred vision, dizziness, easy bruising or bleeding, itching, muscle
pain or cramps, rash, and yellowing of the skin or eyes.
Educated patient on new medication Cyproheptadine hydrochloride (CH) is a first-generation antihistamine which is used as an appetite stimulant.Side effects of Cyproheptadine may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:dry mouth, nose, and throat, drowsiness, dizziness, nausea, chest congestion, headache, excitement (especially in children), muscle
