Instructed patient on Prednisone which is used for allergy reactions, rheumatoid arthritis among other diseases. Side effects include: swelling in hands, face and feet, increased hunger, muscle weakness and abdominal distention. One interaction of Prednisone is it may cause hypernatremia, hypokalemia, fluid retention, and elevation in blood pressure.

High blood pressure Teaching 2627

SN instructed patient on Nifedipine. You can take nifedipine before or after meals. Do not bite, chew or break the tablets/capsules. When you first start taking nifedipine you may get a headache or feel flushed. These symptoms usually go after a few days.

Seroquel Teaching 1338

Seroquel is associated with an increased risk of developing high blood sugar, which on rare occasions can lead to coma or death. See your doctor right away if you develop signs of high blood sugar, including dry mouth, unusual thirst, increased urination, and tiredness. If you have diabetes or have a high risk of developing it, see your doctor regularly for blood sugar testing

SN instructed patient about Glimepiride is an oral blood sugar-lowering drug in a class of medicines for controlling diabetes. Glimepiride lowers the sugar level in the blood by stimulating insulin to be secreted from the pancreas into the blood. Nausea and upset stomach may occur.

Xarelto Teaching 1956

SN instructed patient that the Xarelto is used to prevent or treat a type of blood clot called deep vein thrombosis ( DVT ), which can lead to blood clots in the lungs ( pulmonary embolism ). A DVT can occur after certain types of surgery. Xarelto is also used in people with atrial fibrillation ( a heart rhythm disorder ) to lower the risk of stroke caused by a blood clot. If you take Xarelto 1 time each day: Take the missed dose as soon as you remember. Take your next dose the following day and stay on your once-daily schedule. Do not take extra medicine to make up the missed dose.

Coumadin Teaching 2060

SN instructed patient / caregiver regarding medication Coumadin / Warfarin. SN explained to patient / caregiver that Coumadin / Warfarin is an anticoagulant which is prescribed to people with an increased tendency for thrombosis or as secondary prophylaxis ( prevention of further episodes ) in those individuals that have already formed a blood clot ( thrombus ). Explained that Warfarin treatment can help prevent formation of future blood clots and help reduce the risk of embolism. Informed that Warfarin will prolong bleeding time, instructed to avoid activities that increase risks of trauma. SN explained to patient / caregiver that the adverse effects of this medication includes: hemorrhage, nosebleeds and blood in urine or stool ( black tarry stools ). Instructed to notify physician if experiencing unusual bleeding while on this medication.

Humalog Teaching 2160

SN instructed patient on Humalog 75-25, stated that it is used along with a proper diet and exercise program to control high blood sugar in people with diabetes. This product is a combination of two man - made insulins: intermediate - acting insulin lispro protamine and rapid - acting insulin lispro. This combination starts working faster and lasts for a longer time than regular insulin. Insulin is a natural substance that allows the body to properly use sugar from the diet. It replaces the insulin that your body no longer produces, thereby lowering your blood sugar. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke. SN also instructed that pain, redness, swelling or itching at the injection site may occur. These effects usually go away after a few days or weeks. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

SN instructed patient and caregiver regarding new medication Entresto. This medication is used to treat certain types of heart failure. It may help you live longer and lower your chance of having to go to the hospital for heart failure. This product contains 2 medications: sacubitril and valsartan. They work by relaxing blood vessels so that blood can flow more easily, which makes it easier for your heart to pump blood to your body. Cough, dizziness, or lightheadedness may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose. Take your next dose at the regular time. Do not double the dose to catch up.

Entresto is used to treat certain types of heart failure. It will help you live longer and stay out of the hospital for heart failure. Entresto contains 2 medications: sacubitril and valsartan. They work by relaxing blood vessels so that blood can flow more easily, which makes it easier for your heart to pump blood to your body. Common SE: Coughing, dizziness, or lightheadedness, if this persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. Take medication as prescribed to get the better manage the disease process of Heart Failure.
