SN instructed patient / caregiver on Zofran - ondansetron. This medication blocks the actions of chemicals in the body that can trigger nausea and vomiting. Zofran is used to prevent nausea and vomiting that may be caused by surgery, cancer chemotherapy, or radiation treatment. Serious side effects include blurred vision or temporary vision loss ( lasting from only a few minutes to several hours ), slow heart rate, trouble breathing, anxiety, agitation, shivering, feeling like you might pass out, and urinating less than usual or not at all. Stop taking Zofran and call your doctor at once if you have any of these side effects. Ondansetron may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert. Zofran side effects get emergency
medical help if you have signs of an allergic reaction to Zofran: rash, hives, fever, chills, difficult breathing, swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: severe constipation, stomach pain, or bloating, headache with chest pain and severe dizziness, fainting, fast or pounding heartbeats, fast or pounding heartbeats, jaundice ( yellowing of the skin or eyes ); blurred vision or temporary vision loss ( lasting from only a few minutes to several hours ); high levels of serotonin in the body - agitation, hallucinations, fever, fast heart rate, overactive reflexes, nausea, vomiting, diarrhea, loss of coordination, fainting. Patient / caregiver verbalized understanding. Instruct to call agency for questions or concerns.
SN instructed patient regarding new medication Tamsulosin. This medication is used to relieve the symptoms of benign prostatic hyperplasia ( BPH ) such as difficult urination and inability to empty your bladder completely, common symptoms of enlarged prostate. Tamsulosin works by relaxing bladder neck muscles and muscle fibers in the prostate itself and make it easier to urinate. Common side effects include dizziness, nasal congestion, rhinitis, rhinorrhea, sinus congestion and seasonal allergic rhinitis, drowsiness. Take this medication at bedtime to avoid becoming sleepy during the day. This medication may also cause you to become dizzy when standing up from a sitting or lying position, so rise slowly. Call 911 or get immediate emergency
help if you have any signs of allergic reaction such as blistering rash, swelling of face; tongue, throat, or difficulty breathing. Patient verbalized understanding of all teaching.
Patient and CG instructed on Pravastatin. Pravastatin is used to lower cholesterol and triglycerides (types of fat) in the blood. Pravastatin is also used to lower the risk of stroke, heart attack, and other heart complications in people with or without coronary heart disease or other risk factors. Pravastatin can cause a condition that results in the breakdown of skeletal muscle tissue, leading to kidney failure. This condition may be more likely to occur in older adults and in people who have kidney disease or poorly controlled hypothyroidism (underactive thyroid). Get emergency
medical help if you have signs of an allergic reaction to pravastatin: hives; difficulty breathing; swelling of your face, lips, tongue, or throat. In rare cases, pravastatin can cause a condition that results in the breakdown of skeletal muscle tissue, leading to kidney failure. Stop using and call your doctor at once if you have:unexplained muscle pain, tenderness, or weakness; fever, unusual tiredness; dark colored urine; chest pain; upper stomach pain, loss of appetite; or jaundice (yellowing of the skin or eyes). Common pravastatin side effects may include: muscle or joint pain; nausea, vomiting, diarrhea; headache; or cold symptoms such as stuffy nose, sneezing, sore throat.
SN instructed caregiver that Floranex has been used as a probiotic to aid in digestion, to prevent diarrhea, and to relieve symptoms of irritable bowel syndrome. This medicine may work by helping the body maintain normal consistency of bacteria in the stomach and intestines. You should not use Floranex if you are allergic to soy products or if you are lactose intolerant. To make sure you can safely use Floranex, tell your healthcare provider if you have asthma or allergies. Get emergency
medical help if you have any of these signs of an allergic reaction: hives; chest tightness, difficulty breathing; swelling of your face, lips, tongue, or throat. Less serious side effects may be more likely, and you may have none at all.
SN instructed patient/ caregiver about Gentamicin. SN explained that Gentamicin is an antibiotic that fights bacteria. Gentamicin is used to treat severe or serious bacterial infections. You should not use this medicine if you are allergic to gentamicin or similar antibiotics such as amikacin, kanamycin, neomycin, paromomycin, streptomycin, or tobramycin. Common side effects may include: vision problems; nausea, vomiting, loss of appetite, weight loss; a light-headed feeling, like you might pass out; itching or rash; pain where the medicine was injected; a headache, mood changes; or joint pain. Get emergency

SN instructed patient/ caregiver about Baclofen.That it is a muscle relaxer and an antispastic agent. SN instructed to Get emergency

Call your doctor at once if you have: weak or shallow breathing; confusion, hallucinations; or a seizure (convulsions). Common baclofen side effects may include: drowsiness, dizziness, weakness, tired feeling; headache; sleep problems (insomnia); nausea, constipation; or urinating more often than usual.
SN instructed patient/ CG about Miralax to relieve constipation. SN instructed that MiraLax is used as a laxative to treat occasional constipation or irregular bowel movements. Common MiraLax side effects may include: bloating, gas, upset stomach; dizziness; or increased sweating.Get emergency
medical help if you have any signs of an allergic reaction to MiraLax such as: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop taking this medicine and call your doctor at once if you have: severe or bloody diarrhea; rectal bleeding; blood in your stools; or severe and worsening stomach pain.
SN instructed patient/CG on Acetaminophen pain medication usually used to treat minor to moderate pain. SN instructed patient/CG to never exceed 3000 mg in a 24 hour period. As with any medication Acetaminophen may have side effects, and if you experience SOB or a life threatening side effect please call 911 and seek emergency
treatment.
