SN Instructed patient about the Eliquis ( apixaban ) this is helps to prevent that platelets in your blood
from sticking together and forming a blood
clot. Eliquis is used to lower the risk of stroke caused by a blood
clot in people with a heart rhythm disorder called atrial fibrillation. Because Eliquis keeps your blood
from coagulating ( clotting ) to prevent unwanted blood
clots, this medicine can also make it easier for you to bleed, even from a minor injury such as a fall or a bump on the head. Do not stop taking Eliquis unless your doctor tells you to. Stopping suddenly can increase your risk of blood
clot or stroke
SN instructed patient that Eliquis is used to prevent serious blood
clots from forming due to a certain irregular heartbeat ( atrial fibrillation ) or after hip/knee replacement surgery. With atrial fibrillation, part of the heart does not beat the way it should. This can lead to blood
clots forming, which can travel to other parts of your body (such as the lungs or legs) or increase your risk for stroke. In the United States, apixaban is also approved to treat certain types of blood
clots ( deep vein thrombosis - DVT, pulmonary embolus - PE ) and to prevent them from forming again. Eliquis is an anticoagulant that works by blocking certain clotting proteins in your blood
. SN also instructed that Nausea, easy bruising, or minor bleeding ( such as nosebleed, bleeding from cuts ) may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. This medication can cause serious bleeding if it affects your blood
clotting proteins too much. Tell your doctor right away if you have any signs of serious bleeding, including: unusual pain / swelling / discomfort, unusual bruising, prolonged bleeding from cuts or gums, persistent / frequent nosebleeds, unusually heavy / prolonged menstrual flow, pink/dark urine, coughing up blood
, vomit that is blood
y or looks like coffee grounds, severe headache, dizziness / fainting, unusual or persistent tiredness / weakness, blood
y / black / tarry stools, difficulty swallowing.
SN instructed patient about drug Cilostazol, trade name: Pletal. Is a medication for the treatment of intermittent claudication (pain in your calves when walking), a condition caused by narrowing of the arteries that supply the legs with blood
. Patients with intermittent claudication develop pain when they walk because not enough oxygen-containing blood
reaches the active leg muscles. Cilostazol reduces the pain of intermittent claudication by dilating the arteries, thereby improving the flow of blood
and oxygen to the legs. It also reduces the ability of blood
to clot. Cilostazol enables patients with intermittent claudication to walk longer and faster before developing pain. Cilostazol is a quinolinone-derivative medication that inhibits platelet aggregation and is a direct arterial vasodilator. Its main effects are dilation of the arteries supplying blood
to the legs and decreasing platelet coagulation. Do not stop taking this medication without talking to your doctor.
SN instructed patient about Xarelto (rivaroxaban) is an anticoagulant (blood
thinner) that prevents the formation of blood
clots. Used to treat A-fib and lower the risk of a stroke from a blood
clot. Side effects include bruising, itching, muscle pain, pain in arms or legs. Serious side effects include: urine that looks red, pink, or brown;headache, dizziness, weakness, feeling like you might pass out;blood
y or tarry stools, coughing up blood
or vomit that looks like coffee grounds.
SN instructed patient and caregiver about Clonidine, it is used alone or with other medications to treat high blood
pressure (hypertension). Lowering high blood
pressure helps prevent strokes, heart attacks, and kidney problems. It works by relaxing blood
vessels so blood
can flow more easily. Possible SE/AR may include: dizziness, lightheadedness, drowsiness, dry mouth, or constipation may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. To relieve dry mouth, suck on (sugar
less) hard candy or ice chips, chew (sugar
less) gum, drink water, or use a saliva substitute. Tell your doctor right away if you have any serious side effects, including: fainting, slow/irregular heartbeat, mental/mood changes like, irritability, depression.
Instructed patient on medication Irbesartan, it is used to treat high blood
pressure (hypertension). Lowering high blood
pressure helps prevent strokes, heart attacks, and kidney problems. It works by relaxing blood
vessels so that blood
can flow more easily. Dizziness, lightheadedness, or upset stomach may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position.
Amlodipine is used with or without other medications to treat high blood
pressure. Lowering high blood
pressure helps prevent strokes, heart attacks, and kidney problems. Amlodipine belongs to a class of drugs known as calcium channel blockers. It works by relaxing blood
vessels so blood
can flow more easily. Amlodipine is also used to prevent certain types of chest pain (angina). It may help to increase your ability to exercise and decrease the frequency of angina attacks. It should not be used to treat attacks of chest pain when they occur. Use other medications (such as sublingual nitroglycerin) to relieve attacks of chest pain as directed by your doctor.
Instructed patient Call 911 anytime you think you may need emergency care. For example, call if: You passed out (lost consciousness). You have signs of severe bleeding, such as: A severe headache that is different from past headaches. Vomiting blood
or what looks like coffee grounds. Passing maroon or very blood
y stools. Call your doctor or nurse call line now or seek immediate medical care if: You have unexpected bleeding, including: Blood
in stools or black stools that look like tar. Blood
in your urine. Bruises or blood
spots under the skin. You feel dizzy or light-headed.
SN did head to toe assessment and vital signs monitoring done. SN instructed patient on medication Losartan, it is used to treat high blood
pressure (hypertension).
Instructed patient about Avandia. This is an oral antidiabetic used to decrease blood sugar
levels. Avandia is used in DM type II , alone or in combination of other oral antidiabetic drugs.
