Instructed patient about S/S of possible urinary tract infection, such as, back pain, burning, oliguria, dysuria, retention, fever, etc.
Taught that low back pain and rectal pressure may constitute as signs/symptoms of constipation.
Taught that low back pain and headache may constitute as signs/symptoms of constipation.
Instructed patient when should you contact your caregiver?, The skin around the nephrostomy tube is red, swollen, itches, or has a rash, you have a fever, you have lower back or hip pain, There are changes in how your urine looks or smells, a large amount of urine is draining into the drainage bag over a short period of time.
SN instructed patient and caregiver on hypertensive urgency which is a situation where the blood pressure is severely elevated and that experiencing hypertensive urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or severe weakness, change in vision or difficulty speaking. Patient and caregiver were advised to seek immediately medical assistance and/or call 9-1-1 if any of these signs or symptoms appear. Patient and caregiver verbalized understanding. Family is independent with hypertension process.
RN instructed patient and caregiver on hypertensive urgency which is a situation where the blood pressure is severely elevated and may or may not experience one or more of these symptoms: Severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or weakness of extremities, change in vision and difficulty speaking and immediately ask for medical assistance calling 9-1-1. RN instructed patient and caregiver on no-added-salt or salt-controlled diet that can help control high blood pressure. Both patient and caregiver verbalized understanding.
SN instructed patient that the following are the common symptoms of spinal stenosis: Tingling or numbness in legs or arms, Stiffness and severe back pain, difficulty in walking and standing for long time, might experience balancing problems and resting may provide temporary relief but the symptoms reoccur.
Patient was instructed on how to remove bubble from the syringe. Draw up the insulin slowly and steadily. When bubbles are forming in the syringe means that the drawing has been done too fast, so push the insulin back into the bottle and re-draw. Patient was told to do this many times as needed until the bubbles are gone.
Skilled Nurse instructed caregiver get at least 4 pillows, include one of those long body pillows since you can and place them between the knees, ankles, under the arms and behind the back when the patient is laid on her side.
The patient was instructed in ruptured disk in the use and request of the back brace, support, or belt. The patient was advised in the need for complete bed rest to reduction inflammatory answers and to reduce stress in the spine. The patient was advised to use a urinal, bedpan, or bedside commode during the period of bed rest to evade walking.
