Neuropathy assessment and teaching on management, medication, and alternative therapies to alleviate pain.
SN instructed patient and caregiver that smoking causes cancer, heart disease, stroke, lung diseases, diabetes, and chronic obstructive pulmonary disease (COPD), which includes emphysema and chronic bronchitis. Smoking also increases risk for tuberculosis, certain eye diseases, and problems of the immune system, including rheumatoid arthritis. It is very important for you to quit smoking to prevent chances of getting any or all of the diseases.
Skilled nurse developed patient plan of care with patient/caregiver involvement to be countersigned by physician. SN to perform complete physical assessment each visit with emphasis on disease process. SN to assess other co-morbidities including list diseases and other conditions that present themselves during this episode of care. SN to recognize and intervene to minimize complications; notify physician immediately of any potential problems that impede completion of patient recovery and desired goals.
Instructed patient how prevent a decrease in physical activity. Encourage activity including cooperation with neighboring evacuees, incorporate rehabilitative activity into daily life. Explain the benefits of activities such as walking/ exercise on health, and promote them. Sn leave patient calmly watching TV.
Instruct patient and caregiver regarding self-management of total knee replacement. Instructed patient about how is life different after a knee replacement it’s also normal to have temporary swelling, pressure and/or bruising in your ankles and feet, and these may last for a few months. Here are a couple things you can do to help: ice your knee (always cover ice packs in a towel to avoid frostbite, wear compression stockings, elevate your leg with a pillow, ankle exercises verbalized.
SN instructed caregiver that the medical home can schedule health maintenance visits frequently enough to be proactive about new issues, ensure that the family has access to reliable information, community services, and resources and coordinate care and interpret information or advice from specialists
SN instructed the patient to keep oxygen away from any open flame to include smoking and also in-home pilot lights such as stove, furnace and water heater. Oxygen produced from concentrator and oxygen tanks is flammable. Call your company for a humidifier if causing nasal dryness or you may purchase lubricants made for oxygen use at your local pharmacy. Do not use Vaseline or petroleum based products as these can cause irritation and skin complications. Post no smoking / no Open Flame signs.
