SN advised patient on some serious adverse effect of Lovenox such as: atrial fibrillation, heart failure, skin necrosis, hemorrhage, intracranial hemorrhage, paraplegia, and pneumonia.
Isosorbide is used to prevent chest pain (angina) and reduce strain on the heart in patients with heart disease (coronary artery disease). Isosorbide mononitrate relaxes and widens blood vessels so blood can flow more easily. This medication will not relieve chest pain once it occurs. This medication may also be used in addition to other drugs to improve the symptoms of congestive heart failure (e.g., trouble breathing).
Patient was instructed on some potential complications of hypertension, such as: retinal damage , cerebrovascular accident (CVA) or stroke and kidney failure. Also the patient was instructed on some measures aimed to managing/controlling hypertension, such as: exercise regularly and have regular check-ups, achieve and/or maintain ideal weight, and decrease sodium intake to decrease retention of fluid and the workload of the heart as directed by MD. Patient understood instructions given.
SN instructed patient that people with heart and/or lung failure can benefit from energy conservation techniques like pace activities and spread them throughout the day/week, plan the day in advance so that you won’t feel rushed, break up tasks into small steps and rest in between, sit down instead of standing for most any activity, and work with arms below the shoulders as overhead activities are more taxing.
Carvedilol is used to treat heart failure (condition in which the heart cannot pump enough blood to all parts of the body) and high blood pressure. It also is used to treat people whose hearts cannot pump blood well as a result of a heart attack. Tell your doctor if any of these symptoms are severe or do not go away:tiredness, weakness, lightheadedness, dizziness, headache.

It is also used to treat swelling (edema) caused by certain conditions (e.g., congestive heart failure) by removing excess fluid and improving symptoms such as breathing problems. Instructed that common side effects include, Drowsiness, dizziness, lightheadedness, stomach upset, diarrhea, nausea, vomiting, or headache.
