The patient was instructed in renal transplant in the importance of all-time immunosuppressant management. The patient was taught in the wound care and dressing change. The patient was advised in the need of evade contact to multitudes and persons with known supposed infections. The patient was recommended in the need of recording daily weight at the same time, with the same clothing. The patient was reviewed in taking and recording temperature, pulse, and blood pressure.
Instructed caregiver that treatment includes proper positioning, always avoid placing any weight or pressure on the wound site.
Instructed caregiver A.L.F's staff on relieving and preventing patient's leg edema. The first line of defense is: leg elevation. Elevate legs above the level of the heart which puts minimal pressure on the back of the knees and thighs and lower back. Other help to decrease swelling is limiting salt intake, drink plenty of water, avoid sitting with the feet dependent.
SN instructed pt on medication Lisinopril 20mg PO daily for HTN. Side effects include: dizziness due to a drop in blood pressure. Lisinopril can cause nausea, headaches, anxiety, insomnia, drowsiness, nasal congestion, and sexual dysfunction, and Severe allergic reactions (anaphylaxis) and hives may result. Contact the MD as soon as the above signs/symptoms occur.
Instructed patient all bed-bound and chair-bound persons, or those whose ability to
reposition is impaired, to be at risk for pressure ulcers.
Instructed caregiver how to prevent Pressure Ulcers for Bed bound patients: Take care of the Skin
Inspect the skin at least once every day. Pay attention to any red areas that remain even after
changing position.
Make sure the skin remains clean and dry. Examine the skin daily. Inspect pressure areas gently. Make sure the bed linens remain dry and free of wrinkles. Pat the skin dry, do not rub
Amlodipine is used alone or in combination with other medications to treat high blood pressure and chest pain (angina). SN instructs the patient about the new medication Amlodipine / Benaz may cause dizziness, lightheadedness, or fainting, especially when you get up suddenly from a lying or sitting position, if you feel dizzy, lie down so you do not faint. Then sit for a few moments before standing to prevent the dizziness from returning. If you faint, call your doctor right away.
