Skilled nurse advised patient to elevate both legs to decrease pain and improve circulation.
The patient was advised in pain management in arthritis applying heat or cold to a painful joint for temporary relief avoiding temperatures extremes. The patient was recommended to maintain a regular exercise program that provides stretching and joint mobility. The patient was encouraged to coordinate pain medication with activities. The patient was recommended to use assistive and/or protective devices, as needed, to support affected joints. The patient was advised in the importance of maintaining a safe home environment to prevent falls (removal of small rugs, handrails in showers, tubs, or toilets etc. The patient was encouraged that eating well-balanced diet.
The patient was advised to avoid lifting/stooping in order to decrease back pain. The patient was encouraged to avoid or stop smoking (nicotine decreases diskal circulation).
Assess patient's pain and instructed her relieved with medications and rest.
The patient was instructed in endometriosis and to deal with a future hysterectomy. The patient was taught pain management offering alternative methods to deal with chronic pain
Tramadol is used to relieve moderate to moderately severe pain. If you are having surgery, including dental surgery, tell the doctor or dentist that you are taking tramadol. Side effects can this medication cause: Tramadol may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away: dizziness, weakness, sleepiness,difficulty falling asleep or staying sleep,headache.
SN instructed patient about ways to relieve muscle stiffness such as performing exercises including gentle muscle stretching as tolerated, obtaining adequate rest or uninterrupted sleep in order to reduce stress, massaging affected body parts if not contraindicated and to avoid lifting heavy objects. Instructed to inform doctor or skilled nurse if discomfort progress or when severe or unexplained pain is felt.
SN completed assessment done on all body systems and noted patient with elevated blood pressure during visit. SN completed treatment during visit and noted no drainage on old tx, wound callused and new area found to left medial top of foot remains intact with no drainage noted. SN noted patient complaint of pain to bilateral lower extremities with +2 edema noted. SN educated primary caregiver on the importance of elevation of bilateral lower extremities as well as pain management for patient.
Instructed in new medication Advair diskus to manage asthma. In addition, warned of possible S/E such as sleep disorders, tremor, hypnagogic effects, compressed nerve syndromes, headache, pain, palpitations, pharyngitis, sinusitis, hoarseness (or dysphonia), rhinorrhea, rhinitis, sneezing, nasal irritation, blood in nasal mucosa, keratitis, conjunctivitis, eye redness, viral eye infections, congestion, nausea, vomiting, abdominal pain and discomfort, diarrhea, gastroenteritis, oral discomfort and pain, constipation, oral ulcerations, oral erythema and rashes, oral candidiasis, appendicitis, dental discomfort and pain, unusual taste, muscular pain, arthralgia, articular rheumatism, muscle stiffness, tightness, rigidity, bone and cartilage disorders, upper respiratory tract infection, upper respiratory tract inflammation, lower respiratory tract viral infection, bronchitis, cough, pneumonia, urticaria, flakiness, disorders of sweat and sebum, sweating, hives, viral infections, chest symptoms, fluid retention, bacterial infections, allergic reactions.
Serious side effects of Actonel are chest pain, difficulty or pain when swallowing, pain or burning under the ribs or in the back, new or worsening heartburn, severe joint, bone, or muscle pain, or jaw pain, numbness, or swelling. Stop taking the medication and call the doctor immediately if any of these side effects occur.
