SN instructed patient / caregiver on medication Hydrochlorothiazide. SN explained to patient that Hydrochlorothiazide ( HCTZ ) is a medication indicated for edema and hypertension. SN explained to patient / caregiver that it is a diuretic that reduces fluid volume overload which, in turn, lowers blood pressure. Explained that side effects of this medication includes dizziness, dehydration, weakness, pancreatitis, anemia and nausea. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.
SN instructed patient / caregiver regarding medication Levaquin. SN taught patient / caregiver that levaquin is an antibiotic and that it is used to treat bacterial infections. SN explained to patient that taking levaquin can make the skin more sensitive to sunlight and also may cause swelling or tearing of a tendon. SN explained to patient / caregiver that the side effects of this medication includes headache, nausea, constipation, diarrhea, difficulty sleeping, dry mouth and ear problems. SN instructed patient to notify physician ( MD ) if experiencing persistence or worsening of side effects.
SN instructed patient / caregiver regarding medication Metoprolol. Metoprolol is used for treating high blood pressure, heart pain, abnormal rhythms of the heart, and some neurologic conditions; it reduces the force of contraction of heart muscle and thereby lowers blood pressure. Side effects to report to physician ( MD ) when taking Metoprolol including diarrhea, constipation, fatigue, insomnia, nausea, depression, memory loss, fever, lightheadedness, slow heart rate, low blood pressure, cold extremities, sore throat, and shortness of breath or wheezing. If experiencing any of these side effects or when systolic BP is >160 or < 90 and / or diastolic BP is > 90 or < 60 notify physician ( MD )
SN instruct the patient the following symptoms could be signs of a heart attack: chest discomfort, discomfort in one or both arms, back, neck, jaw, stomach, shortness of breath, cold sweat, nausea, or dizziness. Call 911 for emergent care if you experience any of these s / s ( symptoms / signs ).
SN instructed patient that fluid restrictions are usually necessary when dialysis is initiated, especially if dialysis only occurs three days a week and if urine production is decreased. Build up of fluid can lead to shortness of breath, swelling, and high blood pressure. There is a limit to the amount of fluid that can be safely removed during dialysis. If fluid limits are exceeded and extra water must be removed, negative effects such as muscle cramping, low blood pressure leading to nausea, weakness, dizziness, and possibly extra dialysis sessions to remove the fluid.

It is often used in people in whom high doses of furosemide are ineffective. Instructed that is used to reduce extra fluid in the body ( edema ) caused by conditions such as heart failure, liver disease, and kidney disease. This can lessen symptoms such as shortness of breath and swelling in your arms, legs, and abdomen. Bumetanide is a “ water pill ” ( diuretic ) that causes you to make more urine. This helps your body get rid of extra water and salt. Dizziness may occur as your body adjusts to the medication. Tell your doctor or pharmacist promptly, If this effect persists or worsens. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. Many people using this medication do not have serious side effects. It may cause a serious loss of body water ( dehydration ) and salt / minerals. Tell your doctor right away if you have any of these unlikely but serious side effects such as: muscle cramps, weakness, unusual tiredness, confusion, severe dizziness, fainting, drowsiness, unusual dry mouth / thirst, nausea, vomiting, fast / irregular heartbeat, unusual decrease in the amount of urine.

Reviewed symptoms / signs of hypoglycemia: headache, hunger, sweating, confusion, irritability, dizziness, or feeling shaky. Instructed on common side effects: mild nausea, diarrhea, constipation, dizziness, drowsiness or skin rash, redness or itching. Instructed patient to call 911 in the event of hives; difficulty breathing, swelling of your face, lips, tongue or throat.
SN instructed patient that Bactrim DS is an antibiotic used to treat Urinary tract Infection ( UTI ). Instructed to use this medication for the full prescribed length of time. Your symptoms may improve before the infection is completely cleared. Skipping doses may also increase your risk of further infection that is resistant to antibiotics. Instructed the most common side effects have included nausea, vomiting, anorexia, rash, and urticaria. Instructed to notify physician ( MD ) of any side effects.
SN instructed patient that Fluticasone is used to relieve seasonal and year-round allergic and non - allergic nasal symptoms, such as stuffy / runny nose, itching, and sneezing. It can also help relieve allergy eye symptoms such as itchy, watery eyes. This medication belongs to a class of drugs known as corticosteroids. It works in your nose to block the effects of substances that cause allergies ( such as pollen, pet dander, dust mites, mold ) and to reduce swelling. SN instructed that Nasal dryness / irritation, nausea, or vomiting may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly.
