may be check at different times of the day to get an idea of how well your treatment program is working for you. Typical times to check are before breakfast, before lunch, before dinner, and before bedtime. Sometimes it is helpful to check blood
glucose one or two hours after a meal to see the effect of food on your glucose levels. There are certain times when should check the blood sugar
more often than usual: During periods of illness or stress, when is suspect that blood
glucose is low or high. When there are changes made in the treatment program - such as a change in medication doses, meal plan or activity, when taking new medications.
Patient was instructed on hypoglycemia. Normally, blood
glucose levels increase slightly after you eat a meal. When blood sugar
rises, cells in the pancreas release insulin, causing the body to absorb glucose from the blood
and lowering the blood sugar
level to normal.
Instructed patient use sugar
in moderation. Consider lower sugar
options if available, fruits, vegetables, beans and whole grain foods are good sources of fiber, drink plenty of water, use less salt.
SN instructed patient that the no concentrated sweets (NCS) diet is designed to limit the total sugar
in the diet to achieve and maintain near normal blood sugar
levels. Foods that should be avoided includes sugar
, regular syrup, molasses, and regular jam and jelly, candy, pie, cake, cookies, doughnuts, etc.
SN instructed patient how Lovenox is used to reduce the chance of blood
clots when another blood
thinner is held for a short time. When you take an oral blood
thinner and need a procedure/surgery, your doctor asks you to temporarily stop taking your usual blood
thinner and instead take enoxaparin to help prevent blood
clots before the procedure/surgery. After the procedure/surgery, your doctor will tell you when it is safe to start taking your regularly prescribed oral blood
thinner again. This is called bridge therapy it allows for continued blood
thinning without the blood
being too thin for too long as oral blood
thinners stay in the body for a much longer time.
Patient was instructed on Hyperglycemia. High blood sugar
develops when there is too little insulin or when the body fails to respond properly to the insulin in the blood
. High blood sugar
emergencies often develop because of some underlying condition that may not be obvious, such as a heart attack, infection, or surgery.
Patient was instructed on hypoglycemia. When blood sugar
drops too low, the level of insulin declines and other cells in the pancreas release glucagon, which causes the liver to turn stored glycogen back into glucose and release it into the blood
. This brings blood sugar
levels back up to normal.
SN Instructed patient that should avoid alcohol while on insulin or in any diabetes medications that stimulate the pancreas to produce more insulin, drinking alcohol can cause a dangerous low blood sugar
because your liver has to work to remove the alcohol from your blood
instead of its main job to regulate your blood sugar

daily monitoring on patient's log book, hydration, dietary, and medication compliance. Instructed patient on keeping blood
glucose within normal range and preventing the development of long-term complication. Careful monitoring of diet, exercise and blood sugar
levels are as important as the use of oral medications in preventing complications of diabetes. Also taught infection control, safety/fall precautions and preventions, and to contact MD for any emergency and/or medical concerns and changes.
