Procedures
Instructed patient about the importance of having basic hurricane supplies. Water, can food, yogurt, batteries, and corded phone should be at hand
in case of hurricane ocurrence.
Instructed patient and caregiver about the procedures followed in the case of a hurricane. Should such event occur the Home Health Agency will be on call assisting any patient who has an emergency. Keep Agency phone number at hand

The patient was instructed in carpal tunnel release on care of the hand
s and dressings, to keep the arm and hand
elevated above heart level to reduce swelling and healing. The patient was reviewed to check the dressing daily for signs of bleeding or drainage, when bathing are permitted the patient should be shower with the dressing covered by a plastic bag. The patient was recommended to follow prescribed exercises to maintain circulation and improve range of motion such as sponge squeezing.
Instructed patient / caregiver strenuous work, with hand
cannulated, lifting objects greater than ten pounds, or repetitive motion should all be avoided.
Instructed patient how prevent infections: rub your hand
s together vigorously for at least 15 seconds. Make sure to rub the front and back of each hand
up to the wrist, your fingers and fingernails, between the fingers, and each thumb. Rinse your hand
s with warm water. Dry your hand
s completely with a new, unused paper towel. Don’t use a cloth towel or other reusable towel. These can harbor germs.
Instructed patient always clean your hand
s before and after you come in contact with any part of the PICC line. Your caregivers, family members, and any visitors should use good hand
hygiene, too. Instructed patient keep the PICC dry. The catheter and dressing must stay dry.
SN used hand
cleaner, donned gloves. Drainage bag from old catheter has clear yellow with sediments urine. SN donned sterile gloves, cleaned the perineum around the urinary meatus with chlorhexidine swabs. Flush Foley with 50 cc NS and immediately drained clear yellow urine. Then connected Foley to new drainage bag, then statlock placed on right thigh to secure catheter. Adult diaper put on patient. All items used for procedure disposed of in plastic bag, tied shut and put in household trash.
SN instructed patient the following way you can help prevent an infection wash your hand
s, use soap or an alcohol-based hand
rub to clean your hand
s. Check your skin every day for signs of infection, such as pain, redness, swelling, and oozing. Contact your primary healthcare provider if you see these signs.
