Bleending Teaching 2483

SN instructed patient and caregiver on how to stop a nosebleed. Make a thumbs up with hand on same side as nose bleed. Press side of nose that is bleeding closed and tilt head slightly down to prevent blood from going down into throat. Stay still for 5-10 minutes then gently release. Refrain from blowing nose or putting a tissue in nose x 24 hours. If bleeding does not stop seek medical attention.

High blood pressure Teaching 2000

SN instructed patient about high blood pressure. Normal blood pressure is when your blood pressure is lower than 120 / 80 mm / Hg most of the time. High blood pressure ( hypertension ) is when your blood pressure is 140 / 90 mm / Hg or above most of the time.

Leg edema Teaching 560

Patient was instructed on symptoms of leg edema. If having leg edema feet and legs will initially appear swollen as the day progresses. After a period of time, however, the swelling will set in first thing in the morning and worsen throughout the day. Over time other symptoms will develop: High blood pressure, headaches, increased urination, palpitations, swollen hands and/or wrists, puffy eyes, weight gain.

Patient was instructed on leg edema. The body needs the right amount of blood flow to keep the heart pumping, the legs moving and the brain functioning. Blood circulation, the movement of blood throughout the body, is clearly crucial to the existence, and poor leg circulation can cause many problems, such as leg edema or leg swelling.

Patient was instructed on measures to prevent falls at home during daily activities store household items on lower shelves so there is no need to climb or reach high. Or use a reaching device that you can buy at a medical supply store. To climb for something, use a step stool with handrails.

Foot care Teaching 631

Patient was instructed about the skin damage: the skin may be dry and cracked because of high glucose levels, and cracked skin means it's easier for bacteria to get under the skin and harder for infections to heal. Use a small amount of skin lotion daily, to be sure the feet feel dry - not damp or sticky - afterwards. Try not to get the lotion in between the toes. Keep the toenails trimmed and filed smooth to avoid ingrown toenails. Further teaching is needed.

Perform stretching and relaxation exercises before getting out of bed. Use a tub seat and handheld showerhead when bathing when bathing. Rest before difficult tasks. Take frequent rest periods during the activity. Pace activities, and do not rush. Plan trips before going up and down stairs. Roll, push, or pull instead of lifting. Sit to perform an activity instead of standing, when possible. Hold objects close to you instead of away from your body. Use arm supports to perform an activity (i.e., resting elbows on table while shaving, brushing teeth, and eating). Limit activity on days of high air pollution.

SN instructed patient you can do many things to help knee pain, whether it's due to a recent injury or arthritis you've had for years. Too much rest can weaken your muscles, which can worsen joint pain. Find an exercise program that is safe for your knee. Do exercise, don’t risk a fall. A painful or unstable knee can make a fall more likely, which can cause more knee damage. Curb your risk of falling by making sure your home is well lit, using handrails on staircases, and using a sturdy ladder or foot stool if you need to reach something from a high shelf.
