Digoxin Teaching 1543

Patient was educated about Digoxin 0.125 mg 1 tablet oral daily, side effects may include: fatigue headache, weakness, blurred vision, yellow vision, ARRHYTHMIAS , bradycardia, anorexia , nausea , vomiting , diarrhea, this medication has an action time of 30-120 min, the peak time is about 206 hr and the duration is about 2-4 days.

Patient was instructed on Lorazepam a benzodiazepines a medication used to treat anxiety disorder. Patient was instructed to take medication exactly as indicated by physician and to avoid the consumption of alcohol. Patient was instructed on side effects such as: •drowsiness, dizziness, tiredness;blurred vision;sleep problems (insomnia);muscle weakness, lack of balance or coordination;amnesia or forgetfulness, trouble concentrating;nausea, vomiting,constipation;appetite changes; or skin rash. If side effect become persistent or worsen contact physician immediately.

Omeprazole Teaching 1588

Instructed patient about Omeprazole cap in the most common side effects such as; diarrhea, nausea, vomiting, headaches, rash and dizziness. Nervousness, abnormal heartbeat, muscle pain, weakness, leg cramps, and water retention occur infrequently.

SN instructed patient about Xarelto (rivaroxaban) is an anticoagulant (blood thinner) that prevents the formation of blood clots. Used to treat A-fib and lower the risk of a stroke from a blood clot. Side effects include bruising, itching, muscle pain, pain in arms or legs. Serious side effects include: urine that looks red, pink, or brown;headache, dizziness, weakness, feeling like you might pass out;bloody or tarry stools, coughing up blood or vomit that looks like coffee grounds.

Carvedilol Teaching 1690

Carvedilol is used to treat heart failure (condition in which the heart cannot pump enough blood to all parts of the body) and high blood pressure. It also is used to treat people whose hearts cannot pump blood well as a result of a heart attack. Tell your doctor if any of these symptoms are severe or do not go away:tiredness, weakness, lightheadedness, dizziness, headache.
