SN instructed patient / caregiver regarding medication Lovastatin. SN instructed patient / caregiver that Lovastatin lowers cholesterol by reducing body's production of cholesterol. SN explained to patient / caregiver that the side effects of this medication includes abdominal cramps, elevated liver enzymes, rashes and unexplained muscle pain / tenderness. SN instructed patient / caregiver to notify physician ( MD ) if any of these adverse effects are felt.

Lyrica Teaching 2103

SN instructed patient / caregiver regarding medication Lyrica. SN instructed patient that Lyrica is used to relieve nerve pain as in fibromyalgia and neuralgia. SN explained to patient / caregiver that side effects of this medication includes ataxia, dizziness, peripheral edema, tremors, and weight gain. SN instructed to notify physician ( MD ) if experiencing persistence or worsening of side effects.

Metformin Teaching 2104

SN instructed patient / caregiver regarding medication Metformin. SN explained to patient / caregiver that metformin is an oral anti - diabetic medication and is used to treat type 2 diabetes mellitus. SN explained that it is the first - line drug of choice for the treatment of type 2 diabetes, particularly in overweight and obese people. SN explained that using metformin alone will help to lower blood sugar when it is too high and help restore the way food is used to make energy. SN instructed patient to notify physician ( MD ) if experiencing symptoms / signs ( S / S ) of hypoglycemia such as shakiness, nervousness, sweating associated with dizziness / lightheadedness.

Bumetanide Teaching 2128

SN instructed patient / caregiver that bumetanide ( trade names Bumex or Burinex ) is a loop diuretic to treat heart failure. It is often used in people in whom high doses of furosemide are ineffective. Instructed that is used to reduce extra fluid in the body ( edema ) caused by conditions such as heart failure, liver disease, and kidney disease. This can lessen symptoms such as shortness of breath and swelling in your arms, legs, and abdomen. Bumetanide is a “ water pill ” ( diuretic ) that causes you to make more urine. This helps your body get rid of extra water and salt. Dizziness may occur as your body adjusts to the medication. Tell your doctor or pharmacist promptly, If this effect persists or worsens. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. Many people using this medication do not have serious side effects. It may cause a serious loss of body water ( dehydration ) and salt / minerals. Tell your doctor right away if you have any of these unlikely but serious side effects such as: muscle cramps, weakness, unusual tiredness, confusion, severe dizziness, fainting, drowsiness, unusual dry mouth / thirst, nausea, vomiting, fast / irregular heartbeat, unusual decrease in the amount of urine.

Oxybutynin Teaching 2174

SN instructed patient / caregiver on the medication Oxybutynin. This medication is used to treat overactive bladder a condition in which the bladder muscles contract uncontrollably ( spasms ) and cause frequent urination, an urgent need to urinate, and / or the inability to control urination. Oxybutynin is in a class of medications called anticholinergics. It works by relaxing the bladder muscles. Possible side effects included: Dry mouth or eyes ( less common - dry nose or skin ), dizziness and / or drowsiness, mild constipation, upset stomach, heartburn, nausea, blurred vision or ( eye ) sensitivity to light. Patient / caregiver verbalized understanding.

Pravastatin Teaching 2313

Patient instructed Pravastatin is a medication used to treat high cholesterol. Instructed to follow a low fat diet to help reduce cholesterol. May take with or without food. Medication should be taken in the evening. Grapefruit and grapefruit juice may interact with pravastatin and lead to potentially dangerous effects. Discuss the use of grapefruit products with your doctor.

Levothyroxine Teaching 2320

Patient instructed Levothyroxine is a medication used to treat hypothyroidism. Labs will need to be drawn routinely to check effectiveness & dose may need to be changed. Patient instructed to take this medication 30min. before breakfast & all other meds. Swallow the tablet or capsule whole, with a full glass (8 ounces) of water. The tablet may dissolve very quickly and could swell in your throat. Avoid the following food products, which can make your body absorb less levothyroxine: grapefruit juice, infant soy formula, soybean flour, cotton seed meal, walnuts, and high-fiber foods.

Entresto Teaching 2329

SN instructed patient and caregiver regarding new medication Entresto. This medication is used to treat certain types of heart failure. It may help you live longer and lower your chance of having to go to the hospital for heart failure. This product contains 2 medications: sacubitril and valsartan. They work by relaxing blood vessels so that blood can flow more easily, which makes it easier for your heart to pump blood to your body. Cough, dizziness, or lightheadedness may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. To reduce the risk of dizziness and lightheadedness, get up slowly when rising from a sitting or lying position. If you miss a dose, take it as soon as you remember. If it is near the time of the next dose, skip the missed dose. Take your next dose at the regular time. Do not double the dose to catch up.

Instructed patient on isorsobide which is used to treat and prevent chest pain, side effects of isorsobide are headache, dizziness, lightheadedness, nausea or vomiting. Headache is often a sign that this medication is working; treat it with an aspirin or non-aspirin pain reliever as recommended by your doctor. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Cholesterol Teaching 2420

SN instructed patient on new medication Cholestyramine: This medication is known as a bile acid-binding resin. It works by removing bile acid from the body. In people with high cholesterol, this causes the liver to make more bile acid by using cholesterol in the blood. This helps to lower the cholesterol levels, patient verbalized understanding.
