Azor. Explained that azor is a medication
used to lower blood pressure. Explained that it is a combination medication
of Amlodipine and Olmesartan and it works by dilating the blood vessels causing a decrease in blood pressure. Explained that the side effects include edema, orthostatic hypotension, rash, itching, palpitation and nocturia. SN instructed patient / caregiver to notify physician ( MD ) if experiencing headache, dizziness or flushing.

Benicar. Explained that this medication
is a blood pressure lowering medication
. Explained that the side effects includes hypotension ( low blood pressure ), dizziness, fatigue, headache and /or diarrhea. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.
SN instructed patient / caregiver regarding medication
Dilantin. Explained that Dilantin is an anticonvulsant and is used to control seizures. SN instructed patient / caregiver that the side effects of this medication
includes headache, nausea, vomiting, constipation, dizziness, drowsiness, insomnia and nervousness, swelling of gums may also occur. SN instructed to call physician ( MD ) if patient experiences unusual eye movement, loss of coordination, slurred speech, confusion, blurred vision, or tingling of hands and feet. SN instructed patient / caregiver that blood draws are usually done regularly to make sure that the medication
in the body is within therapeutic level.

Diovan. Explained that diovan is used to treat high blood pressure ( hypertension ) and heart failure. Explained that the high blood pressure reduction helps prevent strokes, heart attacks, and kidney problems and that this drug works by blocking the hormone angiotensin thereby relaxing blood vessels, causing them to widen. SN explained to patient / caregiver that side effects of this medication
includes dizziness, lightheadedness, tiredness, or blurred vision as your body adjusts to the medication
. SN instructed that if any of these effects persist or worsen, to notify doctor immediately.
SN instructed patient / caregiver educated on medication
Ethambutol. SN explained to patient that Ethambutol is an anti - tuberculous medication
indicated for mycobacterial diseases such as Mycobacterium Avium Complex ( MAC ) and is usually used in combination with other anti - mycobacterial anti - infectives. SN explained that the side effects of this medication
includes optic neuritis, hepatitis, abdominal pain, headaches, joint pain, fever, nausea and vomiting. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.

Glimepiride. SN instructed patient / caregiver that Glimepiride is a antidiabetic medication
that lowers blood sugar level. SN instructed patient / caregiver to check blood sugar before taking the medication
to avoid hypoglycemia. SN explained to patient / caregiver that low blood sugar ( hypoglycemia ) can occur during glimepiride therapy. SN explained that the symptoms of low blood sugar include hunger, nausea, tiredness, perspiration, headache, heart palpitations, numbness around the mouth, tingling in the fingers, tremors, muscle weakness, blurred vision, cold temperature, excessive yawning, irritability, confusion, or loss of consciousness. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence of these side effects.
SN instructed patient / caregiver that Hydralazine medication
relaxes and expands blood vessels and is used to treat high blood pressure ( hypertension ). SN explained to patient / caregiver that side effects of this medication
includes: dizziness, drowsiness, headache, constipation, loss of appetite, fatigue and nasal congestion may occur as your body adjusts to the medication
. SN explained to patient / caregiver that to avoid dizziness and lightheadedness when rising from a seated or lying position, get up slowly. SN instructed patient / caregiver to inform doctor if you develop: chest pain, muscle pain, swelling of the hands or feet, yellowing of the eyes / skin, joint pain, a change in the amount of urine. SN explained to patient / caregiver that this drug may cause numbness or tingling of the fingers and toes and that if this occurs, notify doctor. SN explained to patient / caregiver that symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing and should be reported immediately for prompt treatment.

Hydrochlorothiazide. SN explained to patient that Hydrochlorothiazide ( HCTZ ) is a medication
indicated for edema and hypertension. SN explained to patient / caregiver that it is a diuretic that reduces fluid volume overload which, in turn, lowers blood pressure. Explained that side effects of this medication
includes dizziness, dehydration, weakness, pancreatitis, anemia and nausea. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.

Isosorbide. SN explained that Isosorbide is a medication
given to reduce the blood pressure and that it also reduces the heart rate. SN explained that the side effects of this medication
includes fatigue, anxiety, weakness, dizziness, tachycardia, pulmonary edema, constipation and dry mouth. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence of side effects or if experiencing blurred vision, difficulty breathing, wheezing, dizziness, light - headedness, confusion, depression or dry mouth.

Labetalol. SN explained to patient / caregiver that Labetalol is an anti - hypertensive medication
. SN explained that the side effects of this medication
includes fatigue, weakness, anxiety, bradycardia and orthostatic hypotension. SN instructed patient / caregiver to notify physician ( MD ) if experiencing persistence or worsening of side effects.
