Medications
SN instructed patient / caregiver on Zofran - ondansetron. This medication blocks the actions of chemicals in the body that can
trigger nausea and vomiting. Zofran is used to prevent nausea and vomiting that may be caused by surgery, can
cer chemotherapy, or radiation treatment. Serious side effects include blurred vision or temporary vision loss ( lasting from only a few minutes to several hours ), slow heart rate, trouble breathing, anxiety, agitation, shivering, feeling like you might pass out, and urinating less than usual or not at all. Stop taking Zofran and call your doctor at once if you have any of these side effects. Ondansetron may impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert. Zofran side effects get emergency medical help if you have signs of an allergic reaction to Zofran: rash, hives, fever, chills, difficult breathing, swelling of your face, lips, tongue, or throat. Call your doctor at once if you have: severe constipation, stomach pain, or bloating, headache with chest pain and severe dizziness, fainting, fast or pounding heartbeats, fast or pounding heartbeats, jaundice ( yellowing of the skin or eyes
); blurred vision or temporary vision loss ( lasting from only a few minutes to several hours ); high levels of serotonin in the body - agitation, hallucinations, fever, fast heart rate, overactive reflexes, nausea, vomiting, diarrhea, loss of coordination, fainting. Patient / caregiver verbalized understanding. Instruct to call agency for questions or concerns.
SN instructed patient and caregiver on SIMVASTATIN. Simvastatin is a cholesterol-lowering medicine. It inhibits the production of cholesterol by the liver. High blood cholesterol is first treated with exercise, weight loss, and a diet low in cholesterol and saturated fats. When these measures fail, cholesterol-lowering medications such as Simvastatin can
be added. Simvastatin is generally well-tolerated and side effects are rare. Minor side effects of Simvastatin include constipation, diarrhea, fatigue, gas, heartburn, and headache. Major side effects of Simvastatin include abdominal pain or cramps, blurred vision, dizziness, easy bruising or bleeding, itching, muscle pain or cramps, rash, and yellowing of the skin or eyes

Instructed in new medication Tylenol to manage mild pain or fever. In addition, warned of possible S/E such as hemolytuc anemia, neutropenia, leukopenia, pancytopenia, liver damage
, jaundice, hypoglycemia, rash and urticaria. Consult prescriber before giving drug to children younger than age 2. Tylenol is only for short-term use. Consult prescriber if it is given to children for longer than 5 days or adults for longer than 10 days. Instructed not to use for marked fever (higher than 103.1 F), fever persisting longer than 3 days, or recurrent fever unless it is directed by prescriber. Warned that high doses or unsupervised long-term use can
cause hepatic damage
. Excessive ingestion of alcohol may increase the risk of hepatotoxicity. Breast-feeding women: acetaminophen appears in breast milk in low levels (less than 1% of dose). Drug may be used safely if therapy is short-term and does not exceed recommended doses.
SN instructed patient about prednisone is a corticosteroid and prevents the release of substances in the body that cause inflammation. It also suppresses the immune system and can
make you prone to infections. Long term use may lead
to bone loss and you should increase calcium and Vitamin D in your diet. S/E include anxiety, mood changes, irritability, headache, fast heart rate, weight gain, appetite increase, decreased urine.
Patient/caregiver instructed Allopurinol reduces the production of uric acid in your body. Uric acid buildup can
lead
to gout or kidney stones. Take once daily as ordered with a full glass of water. Medication may weaknen your immune system, avoid sick persons. Common side effects are vomiting, diarrhea; drowsiness, headache; changes in your sense of taste; or muscle pain. Notify SN/MD if experiencing bothersome or unusual side effects.
SN instructed patient about poor technique and incorrect landmarking of the injection site can
lead
to site reactions , suboptimal medication absorption and adverse events.

Alcohol may worsen this reaction, and so it should be cautiously consumed. Other common side effects of the combination are diarrhea, constipation and overall pain including
pain in the back, hips and stomach. Less common side effects are breast enlargement and breast pain, which
may be due to the bicalutamide alone.

Do NOT reuse needles or share insulin pens, even if the needle has been changed.The most common side effect of insulin, including
Basaglar, is low blood sugar (hypoglycemia), which
may be serious and life threatening. Signs and symptoms may include dizziness or light-headedness, sweating, confusion, headache, blurred vision, slurred speech, shakiness, fast heartbeat, anxiety, irritability, mood change, or hunger.
