SN instructed patient / caregiver regarding medication Diovan. Explained that diovan is used to treat high blood pressure ( hypertension ) and heart failure. Explained that the high blood pressure reduction helps prevent strokes, heart attacks, and kidney problems and that this drug works by blocking the hormone angiotensin thereby relaxing blood vessels, causing them to widen. SN explained to patient / caregiver that side effects of this medication includes dizziness, lightheadedness, tiredness, or blurred vision as your body adjusts to the medication. SN instructed that if any of these effects persist or worsen, to notify doctor immediately.
SN instructed patient on Humalog 75-25, stated that it is used along with a proper diet and exercise program to control high blood sugar in people with diabetes. This product is a combination of two man - made insulins: intermediate - acting insulin lispro protamine and rapid - acting insulin lispro. This combination starts working faster and lasts for a longer time than regular insulin. Insulin is a natural substance that allows the body to properly use sugar from the diet. It replaces the insulin that your body no longer produces, thereby lowering your blood sugar. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke. SN also instructed that pain, redness, swelling or itching at the injection site may occur. These effects usually go away after a few days or weeks. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

This medication is used to treat high blood pressure and edema ( fluid retention, excess fluid held in body tissues ) caused by various medical problems, including heart, kidney, or liver disease. In addition to taking this medication, making lifestyle changes will also help to control your blood pressure. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking and using alcohol in moderation. To help you remember to take torsemide, take it around the same time every day. Torsemide controls high blood pressure and edema but does not cure these conditions. SN instructed to patient to continue to take torsemide even if you feel well. Do not stop taking torsemide without talking to your doctor. Common side effects of Torsemide includes: Dizziness or lightheadedness when standing or sitting up, excessive urination, constipation, headache and nausea.

It is used along with diet and exercise, and sometimes with other medications, to lower blood sugar levels in people with type 2 diabetes. Empagliflozin is also used to reduce the risk of stroke, heart attack, or death in people who have type 2 diabetes along with heart and blood vessel disease.Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health.This medication may cause changes in your blood sugar. You should know the symptoms of low and high blood sugar and what to do if you have these symptoms. Empagliflozin may cause side effects like urinating a lot, including at night, increased thirst. SN instructed patient about the Eliquis, also known as Apixaban is an anticoagulant. “ANTI” means against and “COAGULANT” refers to the clotting of blood. Apixaban is a medicine ordered by your doctor to prevent harmful blood clots from forming. It does not dissolve existing clots. Sometimes apixaban is referred to as a “blood thinner”, but it does not actually thin your blood. Apixaban is commonly prescribed to people with a heart rhythm problem called atrial fibrillation. Atrial fibrillation increases the risk of a clot forming in the heart. A clot in the heart can dislodge and cause a stroke. The role of apixaban in patients with atrial fibrillation is to prevent formation of a clot in the heart and lower the risk of stroke. Tell your doctor right away if you have these side effects: Unusual bleeding: Vomiting blood or brown material that looks like coffee grounds, Bleeding from cuts that do not stop with pressure, Frequent bleeding from Coughing up blood gums, Nose bleeds, Black or red tarry stools, Bruises that appear without injury, blood in your urine, excessive vaginal bleeding / menstrual flow, Weakness, lightheadedness, dizziness, or fainting Itching or hives, Swelling in your face, hands, mouth, or throat Difficulty breathing or chest tightness.
SN instructed patient about the drug lisinopril is a drug of the angiotensin-converting enzyme inhibitor class used primarily in treatment of hypertension ACE is important because it is an enzyme responsible for producing the chemical, angiotensin II. Angiotensin II causes muscles in most arteries, including the arteries of the heart, to contract, thereby narrowing the arteries and elevating blood pressure. ACE inhibitors such as lisinopril lower blood pressure by reducing the production of angiotensin II, thereby relaxing arterial muscle and enlarging arteries. When the blood pressure is lower, the heart - including the failing heart - does not have to work as hard to pump blood. The arteries supplying the heart with blood also enlarge during treatment with ACE inhibitors.
