Empagliflozin Teaching 2666

SN educated patient and caregiver on medication Empagliflozin (jardiance). It is used along with diet and exercise, and sometimes with other medications, to lower blood sugar levels in people with type 2 diabetes. Empagliflozin is also used to reduce the risk of stroke, heart attack, or death in people who have type 2 diabetes along with heart and blood vessel disease.Taking medication(s), making lifestyle changes (e.g., diet, exercise, quitting smoking), and regularly checking your blood sugar may help to manage your diabetes and improve your health.This medication may cause changes in your blood sugar. You should know the symptoms of low and highblood sugar and what to do if you have these symptoms. Empagliflozin may cause side effects like urinating a lot, including at night, increased thirst. Tell your doctor if any of these symptoms are severe or do not go away:

Pred Forte Teaching 69

Instructed in new medication Pred Forte to manage inflammation of palpebral and bulbar conjunctiva, cornea and anterior segment of globe. In addition, warned of possible S/E, such as, increased intraocular pressure, thinning of cornea, interference with corneal wound healing, increased susceptibility to viral or fungal corneal infections, corneal ulceration, discharge, discomfort, foreign body sensation, glaucoma exacerbation, cataracts, visual acuity and visual field defects, optic nerve damage with excessive or long-term use, systemic effects, and adrenal suppression with excessive or long-term use. Instructed to wash hands before and after installation and warned not to touch tip of dropper to eye or surrounding area. Apply light finger pressure on lacrimal sac for 1 minute after installation. On long-term therapy, have frequent tests of intraocular pressure. Instructed not to share drug, washcloths, or towels with family members, and notify MD if anyone develops same signs or symptoms. Stress importance of compliance with recommended therapy. Notify MD if improvement does not occur within several days or if pain, itching, or swelling of eye occurs. Instructed not to use leftover drug for new eye inflammation because serious problems may occur.

Proper control of diabetes may also lessen your risk of a heart attack or stroke. Metformin works by helping to restore your body's proper response to the insulin you naturally produce. It also decreases the amount of sugar that your liver makes and that your stomach/intestines absorb.

Patient was instructed about Taztia ( Diltiazem ), it belongs to a group of drugs called calcium channel blockers. It works by relaxing the muscles of your heart and blood vessels. It is used to treat hypertension (highblood pressure), angina (chest pain), and certain heart rhythm disorders. Possible SE/AR may include, dizziness. When you sit or lie down for a while, get up slowly to allow your body to adjust and minimize dizziness. Also may also experience bloating, heartburn, muscle cramps, headache, flushing, nasal congestion, sore throat, constipation or diarrhea. Inform your doctor if they become bothersome. Notify your doctor if you develop, breathing difficulties, swelling of the hands or feet, an irregular heartbeat.

Hydralazine Teaching 2092

SN instructed patient / caregiver that Hydralazine medication relaxes and expands blood vessels and is used to treat highblood pressure ( hypertension ). SN explained to patient / caregiver that side effects of this medication includes: dizziness, drowsiness, headache, constipation, loss of appetite, fatigue and nasal congestion may occur as your body adjusts to the medication. SN explained to patient / caregiver that to avoid dizziness and lightheadedness when rising from a seated or lying position, get up slowly. SN instructed patient / caregiver to inform doctor if you develop: chest pain, muscle pain, swelling of the hands or feet, yellowing of the eyes / skin, joint pain, a change in the amount of urine. SN explained to patient / caregiver that this drug may cause numbness or tingling of the fingers and toes and that if this occurs, notify doctor. SN explained to patient / caregiver that symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing and should be reported immediately for prompt treatment.

Metoprolol Teaching 2164

SN instructed patient / caregiver regarding Metoprolol Tartrate, which is in a group of drugs called beta - blockers. That is affect the heart and circulation ( blood flow through arteries and veins ). Metoprolol is used to treat angina ( chest pain ) and hypertension ( highblood pressure ). It is also used to treat or prevent heart attack. Call your doctor at once if you have any of these serious side effects: chest pain, pounding heartbeats or fluttering in your chest, feeling light - headed, fainting; feeling short of breath, even with mild exertion, swelling of your hands or feet, nausea, upper stomach pain, itching, loss of appetite, dark urine, clay - colored stools, jaundice ( yellowing of the skin or eyes ), easy bruising, unusual bleeding ( nose, mouth, vagina, or rectum ), purple or red pinpoint spots under your skin, wheezing, trouble breathing. Less serious side effects may include: dry mouth, constipation, heartburn, vomiting, diarrhea, headache, drowsiness, tired feeling, sleep problems ( insomnia ), or anxiety, nervousness. Do not suddenly stop taking metoprolol tartrate. Sharp chest pain, irregular heartbeat, and sometimes heart attack may occur if you suddenly stop metoprolol tartrate. The risk may be greater if you have certain types of heart disease. Tell your doctor or dentist that you take metoprolol tartrate before you receive any medical or dental care, emergency care or surgery.

Instructed patient in medication Plavix prevent unwanted blood clots that can occur with certain heart or blood vessel conditions. While you take Plavix: you may bruise and bleed more easily, you are more likely to have nose bleeds, and it will take longer for any bleeding to stop. Call your doctor right away if you have any of these signs or symptoms of bleeding: unexpected bleeding or bleeding that lasts a long time, blood in your urine (pink, red or brown urine), red or black stools, bruises that happen without a known cause or get larger cough up blood or blood clots vomit blood or your vomit looks like coffee grounds.
