Patient was educated to follow an adequate diet, high in vegetables cereals and increase the sugar free liquid intake as tolerated and recommended by MD to avoid constipation, also take medication as order, Docusate Ca 240 mg 1 tab PO PRN for constipation.
Instructed in signs and symptoms of lack of oxigen (hypoxemia) such as fast heartrate, rise in pulse and blood pressure, shortness of breath, headache, restlessness and anxiousness, confusion, lethargy, agitation, drop in heartrate and blood pressure as condition progressess, cyansis and others.
Taught that Plavix works by blocking certain blood cells called platelets and prevents them from forming harmful blood clots.
Isosorbide is used to prevent chest pain (angina) and reduce strain on the heart in patients with heart disease (coronary artery disease). SN instructed patient about Aggrenox contains a combination of aspirin and dipyridamole. Aspirin is in a group of drugs called salicylates. It works by reducing substances in the body that cause pain, fever, and inflammation. Dipyridamole keeps platelets in your blood from sticking together to form clots. Aggrenox is used to reduce the risk of stroke in people who have had blood clots or a "mini-stroke" (also called a transient ischemic attack or TIA).Avoid drinking alcohol while you are taking aspirin products. Alcohol may increase your risk of stomach bleeding. Side effects include joint pain, drowsiness, headache, upset stomach, mild heartburn, nausea, vomiting, or diarrhea.
Patient was instructed about Taztia ( Diltiazem ), it belongs to a group of drugs called calcium channel blockers. It works by relaxing the muscles of your heart and blood vessels. It is used to treat hypertension (high blood pressure), angina (chest pain), and certain heart rhythm disorders. Possible SE/AR may include, dizziness. When you sit or lie down for a while, get up slowly to allow your body to adjust and minimize dizziness. Also may also experience bloating, heartburn, muscle cramps, headache, flushing, nasal congestion, sore throat, constipation or diarrhea. Inform your doctor if they become bothersome. Notify your doctor if you develop, breathing difficulties, swelling of the hands or feet, an irregular heartbeat.
SN instructed patient's CG about Quetiapine (Seroquel), it is an atypical antipsychotic approved for the treatment of schizophrenia, bipolar disorder, and along with an antidepressant to treat major depressive disorder. Possible SE/AR may include, dry mouth, dizziness, headache, somnolence. Some common adverse effects, high blood pressure, orthostatic hypotension, high pulse rate, high blood cholesterol, elevated serum triglycerides, abdominal pain, constipation, increased appetite, vomiting, among others.
SN instructed patient that Zocor( Simvastatin) reduces levels of "bad" cholesterol (low-density lipoprotein, or LDL) and triglycerides in the blood, while increasing levels of "good" cholesterol (high-density lipoprotein, or HDL). Zocor is used to lower cholesterol and triglycerides in the blood. It is also used to lower the risk of stroke, heart attack, and other heart complications in people with diabetes, coronary heart disease, or other risk factors.
