Bladder infection Teaching 2042

SN instructed that symptoms of a lower urinary tract infection or bladder infection may include: Frequent need to urinate, burning sensation while urinating pressure in the lower abdomen, pain in the lower back blood in urine. Symptoms of an upper urinary tract infection or a kidney infection may include: Fever, chills, nausea and / or vomiting, Pain higher in the back ( around the upper sides and waist ). In women, the symptoms of a urinary tract infection are similar to those caused by some vaginal infections.

General information Teaching 1653

Patient was given teaching on good body mechanics to avoid injury such as: keep your back straight as you walk and also when lifting making sure to never bend at the waist. If sitting put a pillow or rolled towel to support your lower back.

Leg edema Teaching 2509

Instructed patient in lifestyle and home remedies that may help decrease edema and keep it from coming back. Before trying any self-care techniques, talk to your doctor about which ones are right for you. Moving and using the muscles in the part of your body affected by edema, especially your legs, may help pump the excess fluid back toward your heart. Ask your doctor about exercises you can do that may reduce swelling. Patient verbalized understanding.

Pain relief Teaching 1927

SN instructed patient on nonpharmacologic pain relief measures, including relaxation techniques, massage, stretching, positioning, and hot/cold packs. SN instructed patient to report to physician if patient experiences pain level greater than 6, pain medications not effective, patient unable to tolerate pain medications, pain affecting ability to perform patient’s normal activities.

Sn teaching patient to take pain medication before pain becomes severe to achieve better pain control and decreases the amount of pain medication that is required. SN instructed patient on pain relief measures, including relaxation techniques, massage, stretching, positioning, and / or hot / cold packs.

Migraine Teaching 1925

SN instructed that self-care measures can help ease the pain of a migraine headache. Such as, muscle relaxation exercises. Relaxation may help ease the pain of a migraine headache. SN instructed that relaxation techniques may include progressive muscle relaxation, meditation or yoga. SN also instructed that it is best to get enough sleep, but don't oversleep. Get an adequate amount of sleep each night. It's best to go to bed and wake up at regular times, as well. SN instructed on the importance of being able to rest and relax. If possible, rest in a dark, quiet room when you feel a headache coming on. Place an ice pack wrapped in a cloth on the back of your neck and apply gentle pressure to painful areas on your scalp.

Urinary Tract Infection Teaching 2136

SN instructed that urinary tract infection generally develops when bacteria enter the urinary tract and begin multiplying. The most common urinary tract infections are cystitis, which affects the bladder, and urethritis, which affects the urethra. Urinary tract infections are the second most common type of infection that develop in the body. Symptoms of the infection include burning or pain during urination, an urge to urinate often, pressure in the lower belly, foul-smelling urine, back pain below the ribs, tiredness or shakiness, fever and urine that looks reddish or cloudy. A urinary tract infection is more likely to occur in women than men, and it affects people of all ages.

Instructed patient Keep the dressing (or bandage) clean, dry, and secured to the skin. Do not put lotions or ointments under the dressing. Call your provider right away if you have any of the following: Pain or burning in your shoulder, chest, back, arm, or leg Fever of 100.4?F (38.0?C) or higher Chills Signs of infection at the catheter site (pain, redness, drainage, burning, or stinging)

Instructed patient Exercise Although resting for short periods can alleviate pain, too much rest may actually increase pain and put you at greater risk of injury when you again attempt movement. Research has shown that regular exercise can diminish pain in the long term by improving muscle tone, strength, and flexibility. Exercise may also cause a release of endorphins, the body's natural painkillers. Some exercises are easier for certain chronic pain sufferers to perform than others.

Joint damaged Teaching 2187

SN instructed that the joint that is damaged by injury or disease can be removed and replaced with a new one. There are times when only a part of the joint needs to be replaced or repaired. Your healthcare provider may try other treatments before joint replacement surgery, such as steroid injections or medicines. Pain relief and increased function are the goals of joint replacement. Knee, hip, and shoulder joints are the most common joints replaced. Joints in your elbows, fingers, and ankles can also be repaired or replaced. Your risk of infection, bleeding, and blood clots increase with surgery. You may be allergic to the material used in your new joint. Nerves, muscles, tendons, and blood vessels near your joint may become damaged during surgery. The new joint may loosen or come out of the socket. Sn instructed patient on symptoms / signs ( S/S ) of infection such as fever, drainage, swelling, redness. Patient recalls back partial teaching of redness. Patient instructed to report any symptoms should they occur to physician / nurse ( MD / SN ).
