Diseases Process
SN instructed on arteriosclerosis is hardening and thickening of the walls of the arteries. This condition can occur because of fatty deposits on the inner lining of arteries (atherosclerosis), calcification of the wall of the arteries, or thickening of the muscular wall of the arteries from chronically elevated blood pressure. When arteriosclerosis affects the coronary arteries that supply blood to the heart muscle
, a shortage of oxygen delivered to the heart itself may lead to a heart attack.
Patient has Dx of hypertensive heart disease without heart failure. Hypertensive heart disease refers to heart conditions caused by high blood pressure. The heart working under increased pressure causes some different heart disorders. Hypertensive heart disease can cause thickening of the heart muscle
, coronary artery disease, and other conditions such as heart failure if not controlled. Follow your MD's orders for diet and medications closely
Instructed patient caregiver Loss of appetite, drowsiness, and leg cramps may be signs of sodium loss. Fatigue, muscle
weakness, and shortness of breath may be signs of potassium loss. Dehydration, low sodium, and low potassium can all be dangerous and should be treated right away. Call your doctor or 911 right away if you are dizzy, weak, or having other serious symptoms.
SN instructed patient on ways to conserve energy due to muscle
weakness dx such as small frequent meals, avoiding large meals/overeating, take frequent rest periods to prevent from becoming tired, exercise as often as you can without tiring yourself out, stretching and range of motion is good to help build strength and controlling stress.
Instructed patient that Hypertensive heart disease refers to heart conditions caused by high blood pressure. The heart working under increased pressure causes some different heart disorders. Hypertensive heart disease can lead to complications such as : thickening of the heart muscle
, coronary artery disease, chronic kidney disease and other conditions such as heart failure if not controlled. Follow your MD's orders for Heart Healthy diet, exercise as tolerated and take medications as prescribed.
SN did head to toe assessment and vital signs monitoring done. Received patient sitting on the chair with elevated blood pressure. SN instructed patient on the disease process, and management of CHF. Heart failure occurs when the heart muscle
doesn't pump blood as well as it should. When this happens, blood often backs up and fluid can build up in the lungs, causing shortness of breath. Certain heart conditions gradually leave the heart too weak or stiff to fill and pump blood properly. SN instructed patient to have a healthy lifestyle. Try to lose weight, exercise, use less salt and manage stress. SN will continue to assess and instruct the patient.
