disease without heart
failure. Hypertensive heart
disease refers to heart
conditions caused by high blood pressure. The heart
working under increased pressure causes some different heart
disorders. Hypertensive heart
disease can cause thickening of the heart
muscle, coronary artery disease, and other conditions such as heart
failure if not controlled. Follow your MD's orders for diet and medications closely
Instructed patient that Hypertensive heart
disease refers to heart

disease can lead to complications such as : thickening of the heart
muscle, coronary artery disease, chronic kidney disease and other conditions such as heart
failure if not controlled. Follow your MD's orders for Heart
Healthy diet, exercise as tolerated and take medications as prescribed.
Instructed patient and caregiver on Hypertensive
urgency which is a situation where the blood pressure is severely elevated or higher for your diastolic pressure. That experiencing hypertensive
urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest pain, back pain, numbness/weakness, change in vision, difficulty speaking do not wait to see if your pressure comes down on its own. Seek immediately medical assistance and/or call 9-1-1.
SN instructed patient and caregiver on hypertensive
urgency which is a situation where the blood pressure is severely elevated and that experiencing hypertensive
urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or severe weakness, change in vision or difficulty speaking. Patient and caregiver were advised to seek immediately medical assistance and/or call 9-1-1 if any of these signs or symptoms appear. Patient and caregiver verbalized understanding. Family is independent with hypertension process.
RN instructed patient and caregiver on hypertensive
urgency which is a situation where the blood pressure is severely elevated and may or may not experience one or more of these symptoms: Severe headache, shortness of breath, nosebleeds, and severe anxiety, chest or back pain, numbness or weakness of extremities, change in vision and difficulty speaking and immediately ask for medical assistance calling 9-1-1. RN instructed patient and caregiver on no-added-salt or salt-controlled diet that can help control high blood pressure. Both patient and caregiver verbalized understanding.

can not pump the amount of blood the body needs. Heart
failure does not mean that the heart
has stopped or is about to stop working. It indicates that the heart
can not pump blood the way it should. It can affect one or both sides of the heart
the weakening of the pumping capacity of the heart
causes: Accumulation of blood and fluid in the lungs Accumulation of fluid in the feet, ankles and legs called edema Tiredness and shortness of breath The most common causes of heart
failure are coronary artery disease, high blood pressure and diabetes. It is most common in people over 65, African - Americans, overweight people, and people who have already had a heart
attack. Men are at greater risk than women.

failure, the heart
is not able to pump as well as it should. Blood and fluid may back up into the lungs (congestive heart
failure), and some parts of the body don’t get enough oxygen-rich blood to work normally. These problems lead to the symptoms of heart
failure. Follow a heart
healthy diet. And make sure to limit the salt (sodium) in your diet. Salt causes your body to hold water. This makes your heart
work harder as there is more fluid for the heart
to pump.

rate that can increase your risk of strokes, heart
failure and other heart
-related complications, to prevent atrial fibrillation, it's important to live a heart
-healthy lifestyle to reduce your risk of heart
disease. A healthy lifestyle may include:eating a heart
-healthy diet, increasing your physical activity, avoiding smoking, maintaining a healthy weight, limiting or avoiding caffeine and alcohol, reducing stress, as intense stress and anger can cause heart
rhythm problems, using over-the-counter medications with caution, as some cold and cough medications contain stimulants that may trigger a rapid heart
beat. Patient verbalized.
Patient was instructed on hypertension. Very high blood pressure can cause headache, vision problems, nausea and vomiting. It is called hypertensive
crisis, which is blood pressure that rises very fast.
Instructed on some potential complications of hypertension, such as: hypertensive
crisis (sharp rise in blood pressure to greater than 200/120, severe headache, vomiting), among others.
