Diseases Process
The patient was instructed in hyperparathyroidism in the procedure of assistive devices like walker, cane. The patient was taught to level pain
on a rule and to define pain
so as to improved monitor pain
and pain
killing helpfulness. The patient was encouraged to pain
medication on calendar moderately than as needed. The patient was advised in a plan for using other pain
-relieving ways rather than depend on on pain
medication. The patient was advised to elude using enemas or laxatives.
SN instructed patient that phantom pain
is very real but not everyone who has an amputation has phantom pain
. The pain
may feel like cramping, burning, stabbing or shooting pain
. Some things that can make the pain
feel worse are being too tired, too much pressure on the amputated arm or leg, changes in the weather, stress and infection.
SN instructed patient and caregiver that myocardial infarction and angina pectoris are potential complications of Coronary Artery Disease (CAD). Symptoms for myocardial infarction tightness or pain
in the chest, neck, back
, or arms. Angina symptoms squeezing, pressure, heaviness, tightness, or pain
in the chest. Call your doctor immediately for any symptoms. Both patient and caregiver verbalized understanding.
The patient was advised to avoid lifting/stooping in order to decrease back pain
. The patient was encouraged to avoid or stop smoking (nicotine decreases diskal circulation).
SN observed bag technique and performed proper hand washing per CDC guideline before and after patient contact. Vital signs taken & recorded. Assessed all body systems with focus on urological status. SN educated patient/pcg that benign prostatic hyperplasia (BPH) is a condition where the prostate glands become enlarged which usually happens when a man ages. It may compress the urethra which courses through the center of the prostate. This can impede the flow of urine from the bladder through the urethra to the outside. It can cause urine to back
up in the bladder leading to the need to urinate frequently during the day and night. Signs and symptoms of BPH include hesitant, interrupted, weak stream of urine, urgency and leaking or dribbling, more frequent urination, especially at night. Possible complications of BPH include urinary tract infection and complete blockage of urethra/ SN instructed patient/pcg for patient to increase fluid intake, drink 6-8 glasses of water to prevent the onset of urinary tract infection which is characterized by cloudy urine, fever, foul odor, pain
in bladder area. SN instructed patient to report to SN or MD if any of these symptoms occurs and become bothersome or if pain
persists go to the nearest hospital or ER.
Instructed patient and caregiver on Hypertensive urgency which is a situation where the blood pressure is severely elevated or higher for your diastolic pressure. That experiencing hypertensive urgency may or may not experience one or more of these symptoms: severe headache, shortness of breath, nosebleeds, and severe anxiety, chest pain
, back pain
, numbness/weakness, change in vision, difficulty speaking do not wait to see if your pressure comes down on its own. Seek immediately medical assistance and/or call 9-1-1.
Instructed patient Call your provider right away if you have any of the following: Pain
or burning in your shoulder, chest, back
, arm, or leg Fever of 100.4 F (38.0 C) or higher Chills Signs of infection at the catheter site (pain
, redness, drainage, burning, or stinging)
SN to instruct patient Call your doctor or nurse call line now or seek immediate medical care if: you have new or worse symptoms of a kidney infection. These may include: pain
or burning when you urinate, a frequent need to urinate without being able to pass much urine, pain
in the flank, which is just below the rib cage and above the waist on either side of the back
, blood in the urine.
SN to assess cardiovascular status, identify any signs and symptoms of impaired cardiovascular function. SN to instruct patient on disease process, typical symptoms of an underlying cardiovascular issue include: pain
or pressure in the chest, which may indicate angina, pain
or discomfort in the arms, left shoulder, elbows, jaw, or back
, shortness of breath, nausea and fatigue, lightheadedness or dizziness, cold sweats
Instructed in S/S of possible urinary tract infection such as back pain
, burning, oliguria, dysuria, retention, fever, etc.
