Instructed in new medication Wellbutrin to manage depression. In addition, warned of possible S/E such as headache, seizures, anxiety, confusion, delusions, euphoria, hostility, impaired sleep quality, insomnia, sedation, tremor, akinesia, akathisia, agitation, dizziness, fatigue, syncope, fever, arrhythmias, hypertension, hypotension, palpitations, tachycardia, auditory disturbances, blurred vision, dry mouth, taste disturbance, increased appetite, constipation, dyspepsia, nausea, vomiting, anorexia, diarrhea, impotence, menstrual complaints, urinary frequency, urine retention, weight loss, weight gain, arthritis, pruritus, rash, cutaneous temperature disturbance, excessive diaphoresis, chills and decreased libido. Advised to avoid prolonged sun exposure and to use sun block.
Instructed in foods high in cholesterol such as organ meats, red meats, fatty poultry, shrimp, bacon, cold cuts, hot dogs, saturated fats (found in animal fats and coconut oil), palm kernel, cocoa butter, hydrogenated fats, chocolate, margarine, nondairy substitutes, whole milk, egg yolks, fried foods and others.
Instructed in new medication Atenolol to manage hypertension. In addition, warned of possible S/E such as fatigue, lethargy, vertigo, drowsiness, dizziness, fever, bradycardia, hypotension, heart failure, intermittent claudication, nausea, diarrhea, thrombocytopenia, hyperkalemia, hypoglycemia, increased risk of developing type-two diabetes, dyspnea, bronchospasm, rash and leg pain. Instructed to take drug exactly as prescribed at the same time every day. Do not stop drug suddenly but do call prescriber if unpleasant adverse reactions occur. If pulse rate is below 60 beats/minute, stop taking drug and call prescriber. Women: notify prescriber if pregnancy occurs. Drug usage might be discontinued.
Instructed in measures important in foot care of the diabetic: wear shoes that support and fit properly, wear socks that are clean and fit properly, avoid going barefooted, etc.
Instructed in Vitamin B1 and in S/E such as weakness, restlessness, nauseas, uriticaria, pruritus, tenderness, and ?induration after IM administration. .
Instructed in D/C planning. Instructed to keep MD appointments, take medication exactly as MD ordered and report any S/E and/or S/S to avoid further complications.
Instructed in new medication Tylenol to manage mild pain or fever. In addition, warned of possible S/E such as hemolytuc anemia, neutropenia, leukopenia, pancytopenia, liver damage, jaundice, hypoglycemia, rash and urticaria. Consult prescriber before giving drug to children younger than age 2. Tylenol is only for short-term use. Consult prescriber if it is given to children for longer than 5 days or adults for longer than 10 days. Instructed not to use for marked fever (higher than 103.1 F), fever persisting longer than 3 days, or recurrent fever unless it is directed by prescriber. Warned that high doses or unsupervised long-term use can cause hepatic damage. Excessive ingestion of alcohol may increase the risk of hepatotoxicity. Breast-feeding women: acetaminophen appears in breast milk in low levels (less than 1% of dose). Drug may be used safely if therapy is short-term and does not exceed recommended doses.
Instructed in signs and symptoms of anemia such as headaches, palpitations, paleness of skin, dizziness, increased sensitivity to cold, brittle fingernails and hair, difficulty swallowing, tiredness, etc.
Instructed in possible effects of a stroke such as paralysis or weakness on one side of the body, difficulty with speech and language, urine and fecal incontinence, visual deficits, emotional lability.
Instructed in measures important in foot care of the diabetic: inspect feet dialy, report any foot problems to podiatrist or physician, wash feet dialy with warm soap and water and pat dry; especially between toes.
