Tradjenta Teaching 1908

SN instructed that Tradjenta is used with a proper diet and exercise program and possibly with other medications to control high blood sugar. It is used by people with type 2 diabetes. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke.

Low blood pressure Teaching 1917

SN instructed caregiver that symptoms of low blood pressure may include: blurry vision, confusion, dizziness, fainting (syncope), lightheadedness, nausea or vomiting, sleepiness, weakness. Treatment depends on the cause of your low blood pressure and your symptoms. If any symptoms from a drop in blood pressure, lie patient down right away, then raise patient's feet above heart level.

HTN-stroke Teaching 1920

SN instructed patient / caregiver on factors that increase risk of HTN, such as: age, race, family history, stress, high sodium intake, high cholesterol intake, obesity and sedentary lifestyle. SN instructed patient / caregiver on measures to control/manage HTN, such as: exercise as tolerated, achieve/maintain healthy/ideal weight, decrease sodium intake to decrease fluid retention and the workload of the heart. SN Instructed importance of eating foods rich in calcium, protein and fiber.

Meloxicam Teaching 1922

SN instructed that Meloxicam is a not steroidal anti-inflammatory drug with analgesic and fever reducer effects. Possible can result in gastrointestinal toxicity and bleeding, headaches, rash, and very dark or black stool. Like other NSAIDs, its use is associated with an increased risk of cardiovascular events such as heart attack and stroke. It has fewer gastrointestinal side effects than diclofenac,, piroxicam, and naproxen, and perhaps all other NSAIDs.

Metoprolol Teaching 1931

SN instructed that the metoprolol lowers blood pressure by decreasing workload of heart. Watch for dizziness, lightheadedness when getting up, headache, stomach upset, loss of taste, fatigue and insomnia.

Metformin Teaching 1939

SN instructed that the Metformin is used with a proper diet and exercise program and possibly with other medications to control high blood sugar. It is used in patients with type 2 diabetes. Controlling high blood sugar helps prevent kidney damage, blindness, nerve problems, loss of limbs, and sexual function problems. Proper control of diabetes may also lessen your risk of a heart attack or stroke. Metformin works by helping to restore your body's proper response to the insulin you naturally produce. It also decreases the amount of sugar that your liver makes and that your stomach/intestines absorb.

Ipratropium bromide / albuterol ( Duoneb ) Teaching 1940

SN instructed patient / caregiver about Ipratropium Bromide/Albuterol(Duoneb); get emergency medical help if you have any of these signs of an allergic reaction: hives; difficult breathing; swelling of your face, lips, tongue, or throat. Stop using albuterol and ipratropium and call your doctor at once if you have a serious side effect such as: wheezing, choking, or other breathing problems,chest pain, pounding heartbeats or fluttering in your chest, dangerously high blood pressure (severe headache, anxiety, uneven heart beat), swelling of your ankles or feet, eye pain, or seeing halos around lights, painful or difficult urination. Other common side effects may include: mild headache, or cold symptoms such as stuffy nose, sneezing, cough, or sore throat.

Xarelto Teaching 1956

SN instructed patient that the Xarelto is used to prevent or treat a type of blood clot called deep vein thrombosis ( DVT ), which can lead to blood clots in the lungs ( pulmonary embolism ). A DVT can occur after certain types of surgery. Xarelto is also used in people with atrial fibrillation ( a heart rhythm disorder ) to lower the risk of stroke caused by a blood clot. If you take Xarelto 1 time each day: Take the missed dose as soon as you remember. Take your next dose the following day and stay on your once-daily schedule. Do not take extra medicine to make up the missed dose.

Eliquis Teaching 1957

SN Instructed patient about the Eliquis ( apixaban ) this is helps to prevent that platelets in your blood from sticking together and forming a blood clot. Eliquis is used to lower the risk of stroke caused by a blood clot in people with a heart rhythm disorder called atrial fibrillation. Because Eliquis keeps your blood from coagulating ( clotting ) to prevent unwanted blood clots, this medicine can also make it easier for you to bleed, even from a minor injury such as a fall or a bump on the head. Do not stop taking Eliquis unless your doctor tells you to. Stopping suddenly can increase your risk of blood clot or stroke

Benadryl Teaching 1969

SN instructed patient / caregiver about Diphenhydramine( Benadryl). It is a first-generation antihistamine used to treat a number of conditions including allergic symptoms and itchiness, the common cold, insomnia, motion sickness, and extrapyramidal symptoms. It is a potent anticholinergic agent. This activity is responsible for multiples side effects such as: dry mouth and throat, increased heart rate, pupil dilation, urinary retention, constipation, and, at high doses, hallucinations or delirium. Other side effects include motor impairment, flushed skin, blurred vision at near point owing to lack of accommodation, abnormal sensitivity to bright light (photophobia), sedation, difficulty concentrating, short-term memory loss, visual disturbances, irregular breathing, dizziness, irritability, itchy skin, confusion, increased body temperature, temporary erectile dysfunction, and excitability, and although it can be used to treat nausea, higher doses may cause vomiting. Report any Side effect / Adverse reaction to Physician / Nurse as needed.
